摘　　要：目的了解直肠癌预防性肠造口患者的自我管理能力及希望水平并分析其影响因素。方法采用便利抽样法进行横断面研究,以2022年3—11月在中国医学科学院北京协和医院基本外科接受预防性肠造口的126例直肠癌根治术后患者作为研究对象,收集患者的一般资料,采用问卷调查、量表评价的方式分别了解患者的自我管理能力、希望水平,分析其影响因素。结果患者的自我管理能力问卷总分的平均分为(102.1±12.2)分,处于中等水平;年龄、居住地、术后间隔时间、术后住院时间和主要照顾者均是影响患者得分高低的因素(均P<0.05);年龄、术后间隔时间、术后住院时间、主要照顾者均是得分高低的独立影响因素(均P<0.05)。患者的希望水平量表总分的中位数为34(34,36)分,处于中等水平;年龄、文化程度、术后间隔时间和主要照顾者均是影响患者得分高低的因素(均P<0.05);术后间隔时间是得分高低的独立影响因素(P<0.05)。结论直肠癌预防性肠造口患者的自我管理能力及希望水平均处于中等水平,两者均受到术后间隔时间的影响。患者的年龄、主要照顾者均与患者的自我管理能力及希望水平存在一定关联。Objectives To investigate the self-management ability and level of hope in patients with prophylactic enterostomy for rectal cancer,and analyze the influencing factors.Methods A cross-sectional study was conducted using convenience sam⁃pling.A total of 126 patients who underwent prophylactic enterostomy after radical resection for rectal cancer at the Department of Basic Surgery,Peking Union Medical College Hospital,Chinese Academy of Medical Sciences,from March to November 2022 were recruited.General information of the patients was collected,and a questionnaire and a scale were used to assess their selfmanagement ability and level of hope,respectively.The influencing factors were analyzed.Results The patients’average score on the self-management ability questionnaire with mean(SD)of 102.1(12.2),indicating a moderate level.Age,residence,postop⁃erative time interval,length of hospital stay,and primary caregiver influenced the patients’scores(P<0.05).Age,postoperative time interval,length of hospital stay,and primary caregiver were independent influencing factors of the scores(P<0.05).The medi⁃an score of the level of hope scale for the patients was 34(34,36),indicating a moderate level.Age,education level,postopera⁃tive time interval,and primary caregiver influenced the patients’scores(P<0.05).The postoperative time interval was an indepen⁃dent influencing factor of the scores(P<0.05).Conclusion The self-management ability and level of hope in patients with pro⁃phylactic enterostomy for rectal cancer were at a moderate level,both of which were influenced by the postoperative time interval.Age and primary caregiver were associated with the patients’self-management ability and level of hope.
