摘　　要：目的探讨根尖周囊肿的临床病理特征、诊断及鉴别诊断。方法收集537例根尖周囊肿患者的临床病理资料,对其临床特征及组织学表现进行回顾性分析,并复习相关文献。结果537例根尖周囊肿男性282例,女性255例;发病年龄7~81岁,发生于乳牙者仅2例,上颌前牙受累最多。根尖周囊肿皆内衬复层鳞状上皮,有炎细胞浸润;部分病灶可见出血、钙化、泡沫细胞、牙源性上皮、Rushton小体、胆固醇裂隙以及含铁血黄素等。结论根尖周囊肿乳牙少见,上颌前牙区为好发部位。炎细胞浸润、胆固醇结晶、多核巨细胞、泡沫细胞等炎症表现是根尖周囊肿的重要组织学表现,钙化、牙源性上皮团和Rushton小体等是与其余牙源性囊肿、发育性囊肿的鉴别要点。Purpose To investigate the clinicopathologic features of periapical cysts,diagnosis and differential diagnosis.Methods Clinical and pathological data of 537 periapical cysts patients were collected and retrospectively analyzed in terms of clinical characteristics and histological manifestations,and review relevant literature.Results There were 282 male patients and 255 female patients.The age of onset ranged from 7 to 81 years old and only 2 cases occurred in deciduous teeth.Maxillary anterior teeth were the most affected.Periapical cysts are lined with startified squamous epithelium with inflammatory cell infiltration.Some lesions showed bleeding,calcification,foam cells,odontogenic epithelium.Rushton bodies,cholesterol fissure,and hemosiderin.Conclusion Periapical cysts are rare in deciduous teeth,and maxillary anterior teeth are the best place to occur.Inflammatory cell infiltration,cholesterol crystallization,multinucleated giant cells,foam cells and other inflammatory manifestations are the important histological manifestations of periapical cysts.Calcification,odontogenic epithelium and Rushton corpuscles are the distinguishing points of periapical cysts from other odontogenic cysts and developmental cysts.
