摘　　要：[目的]分析腘窝囊肿接受关节镜下手术的患者采用“4”字体位活瓣切开法的临床有效性和安全性。[方法]回顾性分析2017年1月—2021年7月本院采用4”字体位关节镜下活瓣切开术治疗34例腘窝囊肿患者的临床资料,评价临床及影像结果。[结果]患者均顺利完成手术,围手术期均未发生严重并发症,手术时间平均(41.4±8.4)min,术中失血量平均(20.4±3.3)ml,术后引流量(104.5±29.5)ml,下地行走时间(3.7±0.9)d,住院时间(7.5±1.4)d。所有患者均获随访,随访时间平均(16.3±3.8)个月。随时间推移(术前,术后6个月和末次随访),VAS评分[(6.4±2.5),(3.8±2.4),(0.4±0.1),P<0.05]显著减少,而HSS评分[(43.7±11.4),(73.8±12.6),(96.8±3.5),P<0.05]和膝伸-屈ROM[(82.8±3.3)°,(93.4±4.0)°,(105.9±1.3)°,P<0.05]显著增加。术后6个月,1例复发,囊肿大小3 cm×1 cm,未做治疗。末次随访,2例复发,行开放手术切除。[结论]关节镜下“4”字体位活瓣切开术治疗腘窝囊肿临床效果满意。[Objective]To evaluate the clinical efficacy and safety of arthroscopic valvulotomy in figure-4 position for popliteal cysts.[Methods]A retrospective study was done on 34 patients who received arthroscopic valvulotomy in figure-4 position for popliteal cysts in our hospitals from January 2017 to July 2021.The clinical and imaging data were evaluated.[Results]All patients were operated on smoothly without serious complications,whereas with the operation time of(41.4±8.4)min,intraoperative blood loss of(20.4±3.3)ml,postoperative drainage volume of(104.5±29.5)ml,postoperative walking time of(3.7±0.9)days,and hospital stay of(7.5±1.4)days.All patients were followed up for an average of(16.3±3.8)months.With time elapsed preoperatively,6 months postoperatively and the latest 6 months follow-up,the VAS score[(6.4±2.5),(3.8±2.4),(0.4±0.1),P<0.05]significantly reduced,whereas the HSS score[(43.7±11.4),(73.8±12.6),(96.8±3.5),P<0.05]and knee extension-flexion range of motion(ROM)[(82.8±3.3)°,(93.4±4.0)°,(105.9±1.3)°,P<0.05]significantly increased.At 6 months after surgery,1 patient recurred with a cyst size of 3cm×1cm,without treatment again.At the latest follow-up,2 cases recurred and were excised by open surgery.[Conclusion]The clinical results of arthroscopic valvulotomy in figure-4 position are satisfactory for popliteal cyst.
