齐拉西酮与奥氮平对精神分裂症患者精神症状及糖脂代谢的影响

摘　　要：目的分析齐拉西酮与奥氮平对精神分裂症患者精神症状及糖脂代谢的影响。方法选取2020年4月至2022年12月吉安市第三人民医院收治的精神分裂症患者80例,按随机数字表法分为两组。对照组(40例)患者给予奥氮平口服,观察组(40例)患者服用齐拉西酮,两组均治疗8周。对比治疗前、8周后的疗效指标、糖脂代谢指标、血清催乳素、认知功能变化情况,并统计两组不良反应发生率。结果观察组的阴性症状评分、阳性症状评分、一般精神病理评分、阳性与阴性症状量表(PANSS)总分、临床疗效总评量表(CGI-S)评分、三酰甘油(TG)、空腹血糖(FBS)、低密度脂蛋白(LDL)、空腹胰岛素(INS)、胆固醇(CHOL)、血清催乳素水平均低于对照组,观察组的简易智能精神状态检查量表(MMSE)高于对照组,差异有统计学意义(P<0.05);观察组的不良反应总发生率与对照组相比,差异无统计学意义(P>0.05)。结论相比于奥氮平,齐拉西酮对患者机体内血清催乳素、糖脂代谢水平影响较小,更适合用于治疗精神分裂症患者。Objective To analyze the effects of ziprasidone and olanzapine on psychotic symptoms and glucose and lipid metabolism in patients with schizophrenia.Methods A total of 80 patients with schizophrenia admitted to The Third People's Hospital of Ji'an from April 2020 to December 2022 were selected and divided into two groups according to random number table method.Control group(40 cases)received oral olanzapine,observation group(40 cases)received ziprasidone,both groups were treated for 8 weeks.The efficacy indexes,glucose and lipid metabolism indexes,serum prolactin and cognitive function changes before and 8 weeks after treatment were compared,and the incidence of adverse reactions in the two groups were analyzed.Results In the observation group,negative symptom score,positive symptom score,general psychopathological score,total score of Positive and Negative Symptom Scale(PANSS),total score of Clinical Efficacy Scale(CGI-S),triglyceride(TG),fasting blood glucose(FBS),low density lipoprotein(LDL),fasting insulin(INS),cholesterol(CHOL),serum prolactin,the protein level of the observation group was lower than that of the control group,and the simple Mental State Examination Scale(MMSE)of the observation group was higher than that of the control group,with statistical difference(P<0.05).There was no significant difference in the total incidence of adverse reactions between the observation group and the control group(P>0.05).Conclusion Compared with olanzapine,ziprasidone has less effect on serum prolactin and glucolipid metabolism,which is more suitable for the treatment of schizophrenia.
