摘  要:目的:观察颠倒散对寻常痤疮常见致病菌(表皮葡萄球菌、金黄色葡萄球菌、痤疮丙酸杆菌)的体外抑菌效果,初步探索颠倒散治疗寻常痤疮的作用机制。方法:采用琼脂扩散法,以抑菌圈直径为评价指标,分别测量颠倒散煎液及其稀释液对寻常痤疮常见致病菌的抑菌圈直径;以二倍稀释法分别测量颠倒散煎液及稀释液对寻常痤疮常见致病菌的最小抑菌浓度(Minimum Inhibitory Concentration,MIC))、最小杀菌浓度(Minimum Bactericidal Concentration,MBC)及抑菌浓度指数(Fractional Inhibitory Concentration Index,FICI)。结果:颠倒散对寻常痤疮常见致病菌敏感度各不相同,颠倒散煎液及稀释液均显示对表皮葡萄球菌最敏感,抑菌圈直径分别为(26.6±1.0)mm、(19.8±0.3)mm,其次为痤疮丙酸杆菌,抑菌圈直径分别为(24.0±0.6)mm、(19.5±0.3)mm,再则为金黄色葡萄球菌,抑菌圈直径分别为(20.0±0.8)mm、(15.2±0.5)mm;颠倒散煎液抑菌效果明显优于稀释液,差异具有统计学意义(P<0.05);颠倒散煎液对表皮葡萄球菌、痤疮丙酸杆菌、金黄色葡萄球菌的MIC分别为15.625 mg/mL、31.25 mg/mL、31.25 mg/mL,MBC分别为31.25 mg/mL、62.5 mg/mL、62.5 mg/mL,FICI分别为0.75、1、0.75(FICI≤1)。结论:颠倒散对寻常痤疮常见致病菌(表皮葡萄球菌、金黄色葡萄球菌、痤疮丙酸杆菌)均具有抑菌作用,且颠倒散煎液较稀释液抑菌效果更强;颠倒散中大黄与硫黄两药的作用方式为相加(FICI≤1),两药联合较颠倒散中单味药抑菌效果好。Objective:To observe the in vitro bacteriostatic effect of Diandao San(颠倒散)on common pathogenic bacteria of acne vulgaris(staphylococcus epidermidis,staphylococcus aureus,propionibacterium acnes),preliminary explore Diandao San mechanism for the treatment of acne vulgaris.Methods:Using AGAR diffusion method,bacteriostatic circle diameter as the evaluation index,Diandao San decoction and its dilution on acne vulgaris common pathogenic bacteria of bacteriostatic ring diameterwere measured.The Minimum Inhibitory Concentration(MIC),Minimum Bactericidal Concentration(MBC),and Fractional Inhibitory Concentration Index(FICI)of Diandao San decoctions and diluents against common AV pathogens were measured by double dilution method,respectively.Results:The sensitivity of Diandao San to common pathogens of acne was different,both Diandao San decoction and diluent were most sensitive to staphylococcus epidermidis,bacteriostatic circle diameter were(26.6±1.0)mm、(19.8±0.3)mm,secondly for the acne propionic acid bacillus,bacteriostatic circle diameter were(24.0±0.6)mm、(19.5±0.3)mm.The last is staphylococcus aureus,bacteriostatic circle diameter were(20.0±0.8)mm,(15.2±0.5)mm.The bacteriostatic effect of Diandao San decoction was significantly better than that of diluent,and the MIC of Diandao San against staphylococcus epidermidis,propionibacterium acne and staphylococcus aureus was 15.625 mg/mL,31.25 mg/mL and 31.25 mg/mL,respectively.MBC was 31.25 mg/mL,62.5 mg/mL,62.5 mg/mL,respectively.The FICI was 0.75,1,0.75(FICI≤1).Conclusion:Diandao San has bacteriostatic effect on common pathogens of acne vulgaris,and the bacteriostatic effect of decoction is stronger than that of dilution,and the action mode of Dahuang(Rheum officinale)and Liuhuang(Sulfur grundum)is additive(FICI≤1),and the bacteriostatic effect of the combination of Dahuang and Liuhuang is better than that of single drug.
