基于《素问·咳论》与子午流注理论辨治小儿咳嗽

摘　　要：“五脏六腑皆令人咳,非独肺也”见于《素问·咳论》,此为小儿咳嗽整体发病观的体现。子午流注理论是随中医整体观念应运而生的人体脏腑气血随时间规律变化的学说,与《黄帝内经》中蕴含的“天人合一”观念一脉相承。根据上述理论,可将小儿咳嗽分为晨咳、日咳和夜咳。临证时,若属晨咳之平旦咳,可予三拗汤散寒平喘;若属晨咳之日出咳,可予补肺汤合桃花汤;若属日咳之食时咳,可予旋覆代赭汤;若属日咳之隅中咳,可予二陈汤合三子养亲汤;若属日咳之日中咳,可予泻心汤合泻白散;若属日咳之日昳咳,可予导赤散;若属日咳之哺时咳,可予肾气丸;若属日咳之日入咳,可予七味都气丸;若属夜咳之黄昏咳,可予沙参麦冬汤;若属夜咳之定昏咳,可予小青龙汤;若属夜咳之夜半咳,可予止嗽散合小柴胡汤;若属夜咳之鸡鸣咳,可予黛蛤散合泻青散。The illustration that"All the Zang and Fu organs make people cough,not only the Lung"is seen in Su Wen Ke Lun,which reflects the overall incidence of cough in children.Zi Wu Liu Zhu theory,which is the theory that the Qi and blood of human Zang and Fu change with time,came into being with the overall concept of traditional Chinese medicine,and it is in the same strain as the concept of"harmony between man and nature",which is stated in Huang Di Nei Jing.According to the above theory,children cough can be divided into morning cough,day cough and night cough.In clinical application,if it is Pingdan cough in the morning,San Ao Decoction can be given to dispel cold and relieve asthma;If it is Richu cough in the morning,Bufei Decoction combined with Taohua Decoction can be given;If it is Shishi cough of day cough,Xuanfu Daizhe Decoction can be applied;If it is Ridie cough of day cough,Dao Chi Powder can be used;If it is Yuzhong cough of day cough,Erchen Decoction combined with Sanzi Yangqin Decoction can be applied;If it is Rizhong cough,Xiexin Decoctioncombined with Xiebai Powder can be used;If it is Ridie cough of the day cough,Daochi Powder can be used;If it is Pushi cough of day cough,Shen Qi Pill can be applied;If it is Riru cough of day cough,Qiwei Duqi Pill should be used;If it is Huanghun cough of night cough,Shashen Maidong Decoction can be used;If it is Dinghun cough of night cough,Xiao Qinglong Decoction can be given;If it is a night cough,it can be combined with Xiaochaihu Decoction.If it is Yeban cough of night cough,Zhishou Powder combined with Xiao Chaihu Decoction can be given;If it is Jiming cough of night cough,Daige Powder combined with Xieqing Powder can be applied.
