TCCS血流参数预测急性缺血性脑卒中预后的临床价值

摘　　要：目的:分析经颅彩色多普勒超声(TCCS)血流参数预测急性缺血性脑卒中(AIS)患者预后的临床价值。方法:回顾性分析2017年1月—2022年6月于萍乡市人民医院就诊的AIS患者70例,采用TCCS进行检测,以半年内病情好转情况进行分组,半年内好转为A组(n=42),半年内未好转为B组(n=28)。比较两组患者的颅内段动脉收缩期峰值血流速度(SPV)、舒张末期血流速度(EDV)、平均血流速度(MFV)、搏动指数(PI),并采用ROC曲线获取上述指标的敏感度、特异度及截断值以分析诊断价值。采用logistic回归模型分析评估SPV、EDV、MFV、PI的预测价值。结果:A组SPV、DEV和MFV均显著高于B组,A组PI显著低于B组(P<0.05)。logistic回归分析显示,SPV、EDV、MFV和PI与AIS患者预后均有显著相关性(P<0.05)。由ROC曲线得知,SPV、EDV、MFV和PI均具有预测价值(P<0.05),其中SPV的截断值为52.29 cm/s,EDV的截断值为17.59 cm/s,MFV的截断值为34.76 cm/s,PI的截断值为0.98。结论:TCSS血流参数中的SPV、EDV、MFV和PI能有效预测AIS患者的预后,临床诊治需重视监测上述指标。Objective:To analyze the clinical value of transcranial color coded sonography(TCCS)blood flow parameters on predicting prognosis of patients with acute ischemic stroke(AIS).Method:A total of 70 patients with AIS who were treated in Pingxiang People's Hospital from January 2017 to June 2022 were retrospectively analyzed.TCCS was used to detect the patients,and they were divided into groups according to disease improvement within half a year.The patients who improved within half a year was A group(n=42),and the patients who not improved within half a year was B group(n=28).The medial intracranial artery systolic phase peak blood flow velocity(SPV),end-diastolic blood velocity(EDV),mean blood flow velocity(MFV)and pulsation index(PI)of the two groups were compared,and the sensitivity,specificity and cut off value of the above indexes were obtained by ROC curve to analyze the diagnostic value.The predictive value of SPV,EDV,MFV and PI were evaluated by logistic regression model.Result:SPV,DEV and MFV in A group were significantly higher than those in B group,and PI in A group was significantly lower than that in B group(P<0.05).logistic regression analysis showed that SPV,EDV,MFV and PI were significantly correlated with the prognosis of AIS patients(P<0.05).According to the ROC curve,SPV,EDV,MFV and PI all have predictive value(P<0.05),in which the cut off value of SPV was 52.29 cm/s,the cut off value of EDV was 17.59 cm/s,the cut off value of MFV was 34.76 cm/s,and the cut off value of PI was 0.98.Conclusion:SPV,EDV,MFV and PI in TCSS blood flow parameters can effectively predict the prognosis of AIS patients,and the above indicators should be monitored in clinical diagnosis and treatment.
