和胃化湿汤加减联合叶酸治疗脾虚湿阻型慢性萎缩性胃炎临床研究

摘　　要：目的:观察和胃化湿汤加减联合叶酸治疗脾虚湿阻型慢性萎缩性胃炎的临床疗效及对胃功能、免疫功能的影响。方法:采用随机数字表法将82例脾虚湿阻型慢性萎缩性胃炎患者分为治疗组、对照组各41例。对照组给予叶酸片治疗,治疗组在对照组基础上加用和胃化湿汤加减治疗。比较2组治疗前后中医证候积分、血清胃泌素-17(G-17)、胃蛋白酶原(PG)Ⅰ、PGⅡ、PGⅠ/PGⅡ比值(PGR)和外周血T淋巴细胞亚群(CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))水平,评估2组临床疗效及安全性。结果:治疗组总有效率87.80%,高于对照组68.29%(P<0.05)。2组治疗后血清G-17、PGⅠ、PGR和外周血CD4^(+)、CD4^(+)/CD8^(+)水平较治疗前升高(P<0.05),治疗组治疗后上述指标高于对照组(P<0.05);2组治疗后中医证候积分(主症、次症、总分)、血清PGⅡ、外周血CD8^(+)水平均较治疗前降低(P<0.05),且治疗组治疗后上述指标低于对照组(P<0.05)。2组均未发生不良反应。结论:和胃化湿汤加减联合叶酸治疗脾虚湿阻型慢性萎缩性胃炎疗效确切,能够改善患者胃功能,提高免疫功能,安全性较高。Objective:To observe the clinical effect of modified Hewei Huashi Decoction combined with folic acid for chronic atrophic gastritis of spleen deficiency and dampness obstruction type and its effect on gastric function and immune function.Methods:A total of 82 cases of patients with chronic atrophic gastritis of spleen deficiency and dampness obstruction type were divided into the treatment group and the control group according to the random number table method,with 41 cases in each group.The control group was treated with folic acid tablets,and the treatment group was additionally treated with modified Hewei Huashi Decoction based on the treatment of the control group.Traditional Chinese medicine(TCM)syndrome scores,and the levels of gastrin-17(G-17),pepsinogen(PG)Ⅰ,PGⅡ,and PGⅠ/PGⅡratio(PGR)in serum,and T lymphocyte subsets(CD4^(+),CD8^(+),and CD4^(+)/CD8^(+))in peripheral blood were compared between the two groups before and after treatment.Clinical effects and the safety in the two groups were evaluated.Results:The total effective rate was 87.80%in the treatment group,higher than that of 68.29%in the control group(P<0.05).After treatment,the levels of G-17,PGⅠ,and PGR in serum,and CD4^(+)and CD4^(+)/CD8^(+)in peripheral blood in the two groups were increased when compared with those before treatment(P<0.05),and the above indexes in the treatment group were higher than those in the control group(P<0.05).After treatment,TCM syndrome scores(main symptom,secondary symptom,and the total scores),and the levels of PGⅡin serum and CD8^(+)in peripheral blood in the two groups were decreased when compared with those before treatment(P<0.05),and the above indexes in the treatment group were lower than those in the control group(P<0.05).No adverse responses happened in the two groups.Conclusion:Modified Hewei Huashi Decoction combined with folic acid has a definite curative effect in the treatment of chronic atrophic gastritis of spleen deficiency and dampness obstruction type and can improve gastric function a
