激光辅助次氯酸钠根管消毒清除根管内细菌效果的系统性评价与meta分析

摘　　要：目的 通过meta分析,探讨激光辅助次氯酸钠(sodium hypochlorite, NaClO)根管消毒用于清除根管内细菌的效果。方法 检索Cochrane图书馆、Web of Science、Pub Med、CNKI、维普、万方等数据库,检索时限为建库至2022年7月4日,检索的关键词为激光、根管消毒及随机对照试验。由2名研究人员独立进行文献筛选、资料提取和质量评价,采用Rev Man5.3软件对纳入文献的研究结果进行meta分析。结果 研究最终纳入10篇文献,共426例样本。meta分析结果显示,与NaClO消毒方式相比,激光辅助NaClO根管消毒能降低根管内残留的细菌数量[MD=-0.85,95%CI:(-1.13,-0.57),P<0.01];两组细菌清除率差异有统计学意义[MD=1.92,95%CI:(1.23,2.62),P<0.01]。结论 基于现有研究结果,激光作为辅助手段与NaClO消毒方式结合使用,在清除根管内细菌方面可达到较好的效果。Objective To investigate the effect of laser-assisted sodium hypochlorite(NaClO)root canal disinfection on bacterial removal in root canals through meta-analysis.Methods Cochrane Library,Web of Science,Pub Med,CNKI,VIP,Wanfang and other databases were searched.The search time limit was from the establishment of the database to July 4,2022,and Keywords were laser,root canal disinfection and randomized controlled trials.Literature screening,data extraction and quality evaluation were conducted independently by two researchers,and meta-analysis of the results of the included literature was performed using Rev Man5.3 software.Results In the end,10 articles with a total of 426 samples were included in the study.The results of meta-analysis showed that compared with NaClO disinfection method,laser-assisted NaClO root canal disinfection could reduce the number of residual bacteria in root canals[MD=-0.85,95%CI:(-1.13,-0.57),P<0.01].The bacterial clearance rate of the two groups was statistically significant[MD=1.92,95%CI:(1.23,2.62),P<0.01].Conclusion Based on the existing research results,laser as an auxiliary means combined with NaClO disinfection method can achieve a good effect in removing bacteria in root canals.
