ND∶YAP、ND∶YAG激光联合氟化氨银、牙本质黏结剂、Gluma脱敏剂治疗牙本质敏感的临床研究

摘　　要：目的探讨ND∶YAP、ND∶YAG激光联合38%氟化氨银、牙本质黏结剂、Gluma脱敏剂治疗牙本质敏感的临床效果。方法将120例牙本质敏感患者的330颗牙本质过敏(dentin hypersensitivity,DH)患牙随机分为11个治疗组,其中5组分别进行38%氟化氨银、牙本质黏结剂、Gluma脱敏剂、ND∶YAP激光、ND∶YAG激光脱敏治疗,另外6组进行激光联合治疗,评价各治疗组治疗前及治疗后即刻、治疗后4和8周对温度刺激敏感度的变化,采用视觉模拟量表(visual analogue scale,VAS)对温度敏感度进行检测。结果单独治疗各组术后即刻温度刺激均有改善(P<0.05),治疗效果随时间推移减弱;治疗第8周,氟化氨银组、牙本质黏结剂组、Gluma脱敏剂组完全失效(P<0.05),ND∶YAP激光组、ND∶YAG激光组疗效减弱(P<0.05)。在激光联合治疗中:各组在治疗后3个时间点均有效果(P<0.05);在治疗后第8周,ND∶YAP激光组、ND∶YAG激光组与联合治疗组均维持治疗效果(P<0.05),激光单独治疗组效果弱于联合治疗组(P<0.05)。结论激光联合脱敏剂治疗牙本质敏感具有稳定持久的治疗效果。Objective To investigate the clinical effect of ND∶YAP,ND∶YAG laser combined with 38%silver ammonium fluoride,dentin adhesive agent and Gluma desensitizer in the treatment of dentin hypersensitivity.Methods 330 teeth with dentin hypersensitivity(DH)in 120 patients were randomly divided into 11 treatment groups.5 groups were treated with 38%silver ammonium fluoride,dentin adhesive agent,Gluma desensitizer,ND∶YAP laser,ND∶YAG laser desensitization.The other 6 groups were treated with combined laser therapy.The changes of sensitivity to temperature stimulation in each treatment group were evaluated before treatment,immediately after treatment,4 and 8 weeks after treatment,and the temperature sensitivity was detected by visual analogue scale(VAS).Results The immediate postoperative temperature stimulation was improved in all groups(P<0.05),the therapeutic effect decreased with time,and the curative effect of silver ammonium fluoride group,dentin adhesive agent group and Gluma desensitizer group completely failed at the 8th week(P<0.05),and the curative effect of ND∶YAP laser group and ND∶YAG laser group was weakened(P<0.05).In laser combined therapy:All groups had effects at three time points after treatment(P<0.05);At the 8th week after treatment,ND∶YAP laser group,ND∶YAG laser group and combined treatment group all maintained the therapeutic effect(P<0.05),and the effect of laser alone treatment group was weaker than that of combined treatment group(P<0.05).Conclusion Laser combined with desensitizers has a stable and lasting therapeutic effect on dentin hypersensitivity.
