酒石酸溴莫尼定滴眼液、盐酸卡替洛尔滴眼液治疗原发性开角型青光眼的效果

摘　　要：目的探讨酒石酸溴莫尼定滴眼液、盐酸卡替洛尔滴眼液分别及联合用药对原发性开角型青光眼患者的疗效。方法将2021年1月—2023年1月山东省第一康复医院收治的93例原发性开角型青光眼患者作为研究对象,采用随机数字表法将其分为A组、B组和C组,每组31例。A组采用酒石酸溴莫尼定滴眼液治疗,B组采用盐酸卡替洛尔滴眼液治疗,C组采用二者联合治疗。比较三组眼部血流动力学指标、治疗有效率、视野缺损情况以及不良反应发生率。结果A组总有效率为80.65%,B组总有效率为74.19%,C组总有效率为96.77%,三组比较差异有统计学意义(P<0.05);A、B、C组视网膜中央动脉及睫状后短动脉的舒张末期血流速度(end-diastolic volume,EDV)、收缩期峰值血流速度(peak systolic velocity,PSV)依次升高,而C组阻力指数(resistance index,RI)低于A、B组(P<0.05);治疗后三组视野平均敏感度(mean sensitivity of visual field,MS)均升高,视野平均缺损度(mean defect of visual field,MD)均降低,且C组MS高于A、B组,C组MD低于A、B组(P<0.05);C组不良反应总发生率低于A、B组,但差异无统计学意义(P>0.05)。结论酒石酸溴莫尼定滴眼液、盐酸卡替洛尔滴眼液二者联合用药可有效降低原发性开角型青光眼患者的眼压,改善眼部血流动力学指标以及视野缺损情况,降低不良反应。Objective To investigate the efficacy of bromonidine tartrate eye drops and carteolol hydrochloride eye drops respectively and in combination in patients with primary open-angle glaucoma.Methods A total of 93 patients with primary open-angle glaucoma admitted to the First Rehabilitation Hospital of Shandong from January 2021 to January 2023 were divided into groups A,B and C by random number table method,with 31 cases in each group.Group A was treated with bromonidine tartrate eye drops,group B was treated with carteolol hydrochloride eye drops,and group C was treated with a combination of the two.The hemodynamic indexes,effective rate,visual field defect and incidence of adverse reactions were compared among the 3 groups.Results The total effective rate of group A was 80.65%,group B was 74.19%and group C was 96.77%,the difference among the three groups was statistically significant(P<0.05).The end-diastolic volume(EDV)and peak systolic velocity(PSV)of central retinal artery and posterior short ciliary artery in groups A,B and C increased successively.The resistance index(RI)of central retinal artery and posterior short ciliary artery in group C was lower than that in groups A and B(P<0.05).After treatment,mean sensitivity of visual field(MS)increased and mean defect of visual field(MD)decreased in all three groups.MS in group C was higher than that in groups A and B,and MD in group C was lower than that in groups A and B(P<0.05).The total incidence of adverse reactions in group C was lower than that in groups A and B,but the difference was not statistically significant(P>0.05).Conclusion The combination of brommonidine tartrate eye drops and carteolol hydrochloride eye drops can effectively reduce IOP in patients with primary open-angle glaucoma,improve ocular hemodynamic indexes and visual field defects,and reduce adverse reactions.
