摘 要:目的:分析不同剂量对比剂在头颈部CT血管成像(CTA)中降低静脉伪影的效果。方法:选取2020年12月—2022年12月广西壮族自治区民族医院收治的行头颈部CTA检查的患者100例作为研究对象,依据随机数字表法分为观察组[50例,接受低剂量40 mL(350 mg I/L碘佛醇)对比剂头颈部CTA血管成像检查]与对照组[50例,接受高剂量60 mL(350 mg I/L碘佛醇)对比剂头颈部CTA检查]。比较两组患者动脉显影CT值、静脉残留显影CT值、最终成像质量优良率。结果:两组左颈总动脉、右动脉中动脉M1段、左颈总动脉分叉处、主动脉弓动脉显影CT值比较,差异无统计学意义(P>0.05);观察组左颈内静脉、左锁骨下静脉、上腔静脉、左头臂静脉残留显影CT值低于对照组,差异有统计学意义(P=0.001);观察组最终成像质量优良率高于对照组,差异有统计学意义(P=0.027)。结论:相较于高剂量(60 mL)对比剂头颈部CTA检查,低剂量(40 mL)对比剂头颈部CTA血管成像检查静脉伪影更少,最终成像质量更高,建议临床采纳。Objective:To analyze the effect of different dosages of contrast agents in reducing venous artifacts in head and neck CT angiography(CTA).Methods:One hundred patients admitted to Hospital of Nationalities of Guangxi Zhuang Autonomous Region for head and neck CTA examination from December 2020 to December 2022 were selected as the study subjects.Based on the randomized numerical table method,they were divided into the observation group[fifty cases,receiving low-dose 40 mL(350 mg I/L Ioversol)contrast agent head and neck CTA angiography examination]and the control group[fifty cases,receiving high-dose 60 mL(350 mg I/L Ioversol)contrast agent head and neck CTA examination].The CT values of arterial imaging,CT values of venous residual imaging,and the rate of excellent final imaging quality were compared between the two groups.Results:Comparison of CT values of left common carotid artery,M1 segment of right middle artery,bifurcation of left common carotid artery,and aortic arch arterial imaging in the two groups showed no statistically significant difference(P>0.05);The CT values of the left internal jugular vein,left subclavian vein,superior vena cava,and left cephalic arm vein residual imaging in the observation group were lower than those in the control group,and the difference was statistically significant(P=0.001);The rate of excellent final imaging quality in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.027).Conclusion:Compared with high-dose(60 mL)contrast head and neck CTA,low-dose(40 mL)contrast head and neck CTA angiography has fewer venous artifacts and higher final imaging quality,which is recommended to be adopted by clinical practice.
