黔南地区女性乳腺浸润性导管癌分子分型中上皮间质转化因子表达情况及对预后的价值分析

摘　　要：目的:探究黔南地区的女性乳腺浸润性导管癌分子分型中的上皮间质转化(EMT)因子的表达情况和对预后的价值。方法:选取2018年1月—2022年12月黔南州人民医院收治的女性乳腺癌患者388例作为研究对象,另选取30例女性乳腺癌周围正常乳腺组织作对照,应用免疫组织化学检测法,将患者的癌组织与正常人体的乳腺组织进行比较,探究相关蛋白的表达差异性,分析蛋白表达与患者预后的关系。结果:乳腺浸润性导管癌组织中的Vimentin、MMP2表达阳性率高于正常乳腺腺体组织,E-cadherin表达阳性率低于正常乳腺腺体组织,差异有统计学意义(P<0.05)。本研究随访患者388例,随访率为100.00%;388例患者中,死亡61例,其中60例死于乳腺癌,1例死于其他因素;39例患者在术后发生转移,18例患者术后复发,5例患者在复发的同时出现转移。肿瘤的大小与E-cadherin表达和MMP2表达存在相关性,淋巴结转移与Vimentin表达、E-cadherin表达和MMP2表达均具有相关性,组织学分级与E-cadherin表达和MMP2表达存在相关性,TNM分期与Vimentin表达、E-cadherin表达和MMP2表达均具有相关性。结论:乳腺浸润性导管癌中的相关蛋白表达可实现对乳腺癌的病理分级,对指导临床诊断治疗具有积极意义。Objective:To investigate the expression of epithelial mesenchymal transition(EMT)factor in the molecular staging of female invasive ductal carcinoma of the breast in Qiannan region and its value on prognosis.Methods:Three hundred and eighty-eight cases of female breast cancer patients admitted to the Qiannan Prefecture from January 2018 to December 2022 were selected as the study subjects.Another 30 cases of normal breast tissues around female breast cancer were selected as controls.Immunohistochemical detection method was applied to compare the cancer tissues of patients with normal human breast tissues to explore the differences in the expression of related proteins and analyze the relationship between protein expression and patients'prognosis.Results:The positive rate of Vimentin and MMP2 expression in breast invasive ductal carcinoma tissues was higher than that in normal breast glandular tissues,and the positive rate of E-cadherin expression was lower than that in normal breast glandular tissues,and the difference was statistically significant(P<0.05);In this study,three hundred and eighty-eight patients were followed up,and the follow-up rate was 100.00%;Of the three hundred and eighty-eight patients,sixty-one died,of which sixty died of breast cancer and one died of other factors;Thirty-nine patients developed metastasis after surgery,eighteen patients had postoperative recurrence,and five patients developed metastasis along with recurrence;There was a correlation between tumor size and E-cadherin expression and MMP2 expression;Correlation of lymph node metastasis with Vimentin expression,E-cadherin expression and MMP2 expression;There was a correlation between histologic grading and E-cadherin expression and MMP2 expression,and TNM staging correlated with Vimentin expression,E-cadherin expression and MMP2 expression.Conclusion:The expression of related proteins in invasive ductal carcinoma of the breast can realize the pathological grading of breast cancer,which is of positive significance in guiding clin
