胃镜活检与术后病理检查在胃癌诊断中的一致性、准确性研究

摘　　要：目的 探究胃镜活检与术后病理检查在胃癌诊断中的一致性、准确性。方法 回顾性分析2021年6月至2022年12月于我院行胃部分切除手术的87例胃癌患者临床资料,所有患者均在术前进行胃镜活检和术后病理检查。对比术前胃镜活检与术后病理检查诊断的一致性和准确率。结果 术前胃镜活检检出疑似胃癌、排除胃癌占比率高于术后病理检查,确诊胃癌占比率低于术后病理检查(P<0.05)。术前胃镜活检分化型检出率低于术后病理检查,分化不良型检出率高于术后病理检查(P<0.05)。术前胃镜活检与术后病理检查对胃癌细胞浸润深度的诊断一致率为91.95%(80/87),Kappa一致性指数为0.826(P<0.05)。结论 术前胃镜活检与术后病理检查诊断结果不一致,但术前胃镜活检阳性诊断敏感度较高,可在早期胃癌筛查和诊断预防中发挥重要作用。Objective To explore the consistency and accuracy of gastroscopic biopsy and postoperative pathological examination in the diagnosis of gastric cancer.Methods The clinical data of 87 patients with gastric cancer who underwent partial gastrectomy in our hospital from June 2021 to December 2022 were analyzed retrospectively.The consistency and accuracy of preoperative gastroscopic biopsy and postoperative pathological examination were compared.Results The proportion of suspected and excluded gastric cancer detected by preoperative gastroscopic biopsy was higher than that of postoperative pathological examination,and the proportion of confirmed gastric cancer was lower than that of postoperative pathological examination(P<0.05).The detection rate of differentiated type was lower than that of postoperative pathological examination,and the detection rate of poorly differentiated type was higher than that of postoperative pathological examination(P<0.05).The diagnostic coincidence rate of gastric cancer cell infiltration depth between preoperative gastroscopic biopsy and postoperative pathological examination was 91.95%(80/87),and Kappa consistency index was 0.826(P<0.05).Conclusions The diagnostic results of preoperative gastroscopic biopsy are inconsistent with those of postoperative pathological examination,but the positive diagnostic sensitivity of preoperative gastroscopic biopsy is relatively high,which may play an important role in the screening,diagnosis and prevention of early gastric cancer.
