黄连温胆汤加减联合三联疗法治疗湿热中阻证原发性胆汁反流性胃炎的效果

摘　　要：目的 探讨黄连温胆汤加减联合三疗法治疗湿热中阻证原发性胆汁反流性胃炎的临床效果。方法 收集就诊于我院胃镜室确诊湿热中阻证原发性胆汁反流性胃炎患者80例,平均划分为对照组、试验组。对照组采用三联疗法,服用铝碳酸镁咀嚼、奥美拉唑、莫沙必利,试验组在此方案基础上加服黄连温胆汤;两组疗程均为4周。治疗后,比较两组患者中医证候积分、胃镜疗效、临床疗效。结果 两组患者治疗后主症、次症积分均较治疗前下降,证候均较治疗前缓解(P<0.05);治疗后,试验组主症、次症积分均低于对照组,证候缓解优于对照组(P<0.05)。治疗后,试验组腺体萎缩、胆汁反流改善情况明显优于对照组(P<0.05)。两组患者临床总有效率比较,差异无统计学意义(P>0.05)。结论 黄连温胆汤加减联合三联疗法治疗湿热中阻证原发性胆汁反流性胃炎能显著改善症状,有效对抗胆汁反流。Objective To investigate the clinical effect of Huanglian Wendan decoction in the treatment of damp-heat obstructive primary bile reflux gastritis.Methods A total of 80 patients with primary bile reflux gastritis diagnosed with damp-heat syndrome in the gastroscopy room of our hospital were collected and divided into control group and experimental group.The control group received triple therapy,taking mastication of magnesium aluminocarbonate,omeprazole and mosapride,and the experimental group took Huanglian Wendan decoction on the basis of this regimen;Both groups were treated for 4 weeks.After treatment,TCM syndrome score,gastroscopy effect and clinical effect were compared between the two groups.Results After treatment,the scores of the main and secondary symptoms were decreased,and the symptoms were relieved(P<0.05).After treatment,the scores of main and secondary symptoms in the experimental group were lower than those in the control group,and the remission of syndrome was better than that in the control group(P<0.05).After treatment,the improvement of glandular atrophy and bile reflux in the experimental group was significantly better than that in the control group(P<0.05).There was no significant difference in clinical effective rate between 2 groups(P>0.05).Conclusions The combined therapy of Huanglian Wendan Decoction can significantly improve the symptoms of primary bile reflux gastritis in damp-heat syndrome and effectively combat bile reflux,which is worth popularizing.
