摘　　要：目的:构建坏死性凋亡相关长链非编码RNA(long noncoding RNA,lncRNA)的膀胱尿路上皮细胞癌(urothelial carcinoma of bladder,BLCA)预后模型。方法:从TCGA数据库获取431例BLCA患者的转录组数据和临床数据,并将其按1∶1分为训练组和验证组,结合来自公共数据库的67个坏死性凋亡相关基因,使用R软件进行数据处理和分析,通过Pearson相关性分析、Cox回归分析、LASSO回归模型构建坏死性凋亡相关lncRNA预后模型,运用Kaplan-Meier法进行生存分析,使用接受者操作特征(receiver operator characteristic,ROC)曲线分析评估模型的预测性能,并且进行了免疫微环境分析。结果:成功构建了基于5个坏死性凋亡相关lncRNA的BLCA预后风险模型。生存曲线显示,高风险组的总生存期低于低风险组(P<0.05)。多因素Cox回归分析显示该模型的风险评分可作为BLCA患者总生存期的独立预后因子(P<0.001)。风险评分预测1年、3年和5年生存率的ROC曲线下面积分别为0.685、0.662和0.664。训练组和验证组的结果相互验证了该模型的有效性。肿瘤微环境中,高风险组相对于低风险组具有更明显的免疫浸润状态。结论:成功构建了坏死性凋亡相关lncRNA的BLCA预后模型,该模型可在一定程度上预测BLCA患者的生存率。Objective:To construct a necroptosis-associated long noncoding RNA(lncRNA)prognostic model for urothelial carcinoma of bladder(BLCA).Methods:Transcriptome data and clinical data of 431 BLCA patients were obtained from the TCGA database and divided into training and validation groups on a 1:1 basis,combined with 67 necroptosis-associated genes from public databases,and data were processed and analyzed using the R software.The necroptosis-associated lncRNA prognostic model was constructed by Pearson correlation analysis,Cox regression analysis,and LASSO regression model.Survival analysis was performed using the Kaplan-Meier method.The predictive performance of the model was assessed using receiver operator characteristic(ROC)curve analysis,and an immune microenvironment analysis was performed.Results:A prognostic model of BLCA based on five necroptosis-associated lncRNAs was successfully constructed.Survival curves showed that the overall survival of the high-risk group was lower than that of the low-risk group(P<0.05).Multifactorial Cox regression analysis showed that the risk score of this model could be used as an independent prognostic factor for overall survival of BLCA patients(P<0.001).The area under the ROC curve for the risk score to predict 1-,3-and 5-year survival was 0.685,0.662 and 0.664,respectively.The results of the training and validation groups mutually validated the validity of the model.In the tumor microenvironment,the high-risk group had a more pronounced immune infiltration status relative to the low-risk group.Conclusion:A necroptosis-associated lncRNA prognostic model for BLCA was successfully constructed,and the model can predict the survival rate of BLCA patients to some extent.
