子宫内膜癌病人血清circWHSC1和circPUM1的表达及诊断价值

摘　　要：目的检测子宫内膜癌病人血清circWHSC1和circPUM1表达水平,并探究其在子宫内膜癌中的诊断价值。方法收集2018年6月至2020年6月自贡市第一人民医院120例子宫内膜癌病人、123例子宫内膜增生病人及125例体检健康者分别作为子宫内膜癌组、子宫内膜增生组与对照组。qRT-PCR法测定研究对象血清circWHSC1和circPUM1表达水平,分析血清circWHSC1和circPUM1表达水平与子宫内膜癌病人临床分期、分化程度的相关性,logistic回归分析血清circWHSC1和cir⁃cPUM1表达水平与子宫内膜癌发病的相关性;ROC曲线分析血清circWHSC1和circPUM1表达水平对子宫内膜癌的诊断价值。结果与对照组、子宫内膜增生组相比,子宫内膜癌组病人血清circWHSC1(1.93±0.24比1.02±0.21、1.04±0.19)、circPUM1(1.76±0.25比1.01±0.18、1.02±0.17)表达水平均升高(P<0.05);高分化(G1)组、中分化(G2)组、低分化(G3)组子宫内膜癌病人血清circWHSC1、circPUM1表达水平均依次升高(P<0.05);Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期子宫内膜癌病人血清circWHSC1、circPUM1表达水平均依次升高(P<0.05);circWHSC1、circPUM1表达水平与子宫内膜癌的发病密切相关(OR=2.75、3.71,P<0.05);血清circWHSC1、circPUM1以及联合检测对子宫内膜癌诊断的曲线下面积为0.88(灵敏度为88.2%,特异度为81.5%)、0.82(灵敏度为79.8%,特异度为83.1%)、0.95(灵敏度为89.1%,特异度为93.5%)。结论子宫内膜癌病人血清circWHSC1、circPUM1表达水平均较高,且与病人临床分期和子宫内膜癌的分化程度有关,有望作为筛查子宫内膜癌病人的血清标志物,为子宫内膜癌的早期诊断做贡献。Objective To detect the expression levels of circWHSC1 and circPUM1 in serum of patients with endometrial cancer,and to explore their diagnostic value in endometrial cancer.Methods From June 2018 to June 2020 in the First People's Hospital of Zigong,120 patients with endometrial cancer,123 patients with endometrial hyperplasia,and 125 healthy individuals were collected as endometrial cancer group,endometrial hyperplasia group,endometrial hyperplasia group,and control group,respectively.The expression levels of circWHSC1 and circPUM1 in the serum of the study subjects were determined by qRT-PCR,the correlation between the expression levels of serum circWHSC1 and circPUM1 and the clinical stage and degree of differentiation of endometrial cancer patients was analyzed,the correlation between the expression levels of circWHSC1 and circPUM1 in serum and the incidence of endometrial cancer was analyzed by logistic regression analysis;the diagnostic value of serum circWHSC1 and circPUM1 expression levels for endometrial cancer was analyzed by ROC curve.Results Compared with the control group and the endometrial hyperplasia group,the serum circWHSC1(1.93±0.24 vs.1.02±0.21,1.04±0.19)and circPUM1(1.76±0.25 vs.1.01±0.18,1.02±0.17)expression levels in the endometrial cancer group were increased(P<0.05);the expression levels of serum circWHSC1 and circPUM1 in patients with endometrial cancer in the well differentiated(G1)group,moderately differentiated(G2)group and poorly differentiated(G3)group were increased in turn(P<0.05);the expression levels of serum circWHSC1 and circPUM1 in patients with stageⅠ,Ⅱ,ⅢandⅣendometrial cancer were increased in turn(P<0.05);the expression levels of circWHSC1 and circPUM1 were closely related to the incidence of endometrial cancer(OR=2.75,3.71,P<0.05);the area under the curve of serum circWHSC1,circPUM1 and combined detection in the diagnosis of endometrial cancer was 0.88(sensitivity 88.2%,specificity 81.5%),0.82(sensitivity 79.8%,specificity 83.1%),0.95(sensitivity 89.1%,speci
