中医药辅助治疗肺癌455例疗效观察

摘　　要：目的:观察中医药辅助治疗对肺癌的疗效及生存期的影响。方法:将948例肺癌患者,分为对照组493例和观察组455例;对照组治疗按照《恶性肿瘤中医诊疗指南》推荐的方法;观察组在“指南”治疗的基础上增加中医药辅助治疗,依据辨证分型,分别采用自拟中药化积汤系列方加减,或采用清金消积膏、扶金化积膏治疗。结果:总有效率观察组[80.00%(308/385)]高于对照组[51.84%(240/463)],差异有统计学意义(P<0.05)。观察组癌胚抗原(Carcinoembryonic antigen,CEA)、糖类抗原125(Carbohydrate antigen 125,CA125)水平较对照组明显降低(P<0.05)。观察组(Karnofsky,KPS)60分下降比例比对照组少,差异有统计学意义(P<0.05)。观察组发生不良反应及并发症总数比对照组少,差异有统计学意义(P<0.05)。观察组3年生存率27.98%,对照组为26.95%。观察组平均生存期26.43个月,中位生存期32个月,对照组平均生存期25.04个月,中位生存期27个月;观察组2年生存率53.61%,对照组47.01%。结论:自拟中药化积汤系列方及清金消积膏、扶金化积膏辅助治疗肺癌,具有明显的增效减毒作用,并可改善患者的生活质量,延长生存期。Objective:To observe the influence of adjuvant therapy of TCM on therapeutic effects and survival period of lung cancer.Methods:All 948 patients were randomized into 493 cases of the control group and 455 cases of the observation group;the control group was treated according to the method recommended in TCM diagnosis and treatment guidelines of malignant tumors;on the foundation of the therapy of the guidelines,the observation group added TCM adjuvant therapy,adopting a series of modified prescriptions of selfmade Huaji decoction,or Qingjin Xiaoji ointment,Fujin Huaji ointment respectively in light of syndrome differentiation and treatment.Results:Total effective rate of the observation group was[80.00%(308/385)],higher than[51.84%(240/463)]of the control group,and the difference had statistical meaning(P<0.05).The levels of CEA and CA125 of the observation group reduced remarkably than the control group(P<0.05).The decline ratio of KPS 60 points in the observation group was less than that in the control group,and the difference showed statistical meaning(P<0.05).Total number of adverse reactions and complications in the observation group was less than that in the control group(P<0.05).Three-year survival rate of the observation group was 27.98%,while lower than 26.95%of the control group.Mean survival time of the observation group was 26.43 months,median survival time was 32 months,longer than 25.04 months and 27 months of the control group;two-year survival rate of the observation group was 53.61%,higher than 47.01%of the control group.Conclusion:TCM adjuvant therapy therapy,containing a series of prescriptions of Huaji decoction,Qingjin Xiaoji ointment and Fujin Huaji ointment,in the treatment of lung cancer have significant synergistic and toxicity-reducing effects,which could improve the patients'quality of life and prolong survival period.
