血清miR-326与Th17/Treg细胞在复发缓解型多发性硬化中的表达及其临床意义

摘　　要：目的探究血清miR-326与Th17/Treg细胞在复发缓解型多发性硬化(RRMS)中的表达及其临床意义。方法前瞻性选取2020年1月至2022年12月河北北方学院附属第一医院收治的RRMS患者192例为研究对象,设为研究组,按照患者疾病发展阶段分为急性期107例,缓解期85例,并按2∶1比例选取同期在同院行健康体检者96名为对照组。比较各组miR-326表达与Th17/Treg细胞相关指标的差异,分析miR-326表达与Th17/Treg细胞相关指标预测RRMS发生的临床价值,并分析miR-326表达、Th17/Treg细胞相关指标与病情严重程度间的相关性。结果研究组患者miR-326、白细胞介素(IL)-17A、IL-23水平及Th17百分率均明显高于对照组,IL-35、TGF-β水平及Treg百分率均明显低于对照组,差异均有统计学意义(P<0.05)。急性期患者miR-326、IL-17A、IL-23水平及Th17百分率均明显高于缓解期,IL-35、TGF-β水平及Treg百分率均明显低于缓解期,差异均有统计学意义(P<0.05)。受试者工作特征(ROC)曲线分析显示,miR-326、IL-17A、IL-23、IL-35、TGF-β水平及Th17百分率、Treg百分率预测RRMS发生的曲线下面积(AUC)值分别为0.892、0.888、0.872、0.855、0.805、0.865、0.857,具有较好预测价值(P<0.05)。Pearson分析显示:miR-326、IL-17A、IL-23水平及Th17百分率与病情严重程度呈正相关(P<0.05),IL-35、TGF-β水平及Treg百分率与病情严重程度呈负相关(P<0.05)。Spearman分析显示:miR-326表达与IL-17A、IL-23水平及Th17百分率呈正相关(P<0.05),与IL-35、TGF-β水平及Treg百分率呈负相关(P<0.05)。结论RRMS患者血清miR-326与Th17相关细胞高表达,Treg相关细胞低表达,且血清miR-326、Th17/Treg相关细胞与RRMS病发与病情严重程度相关密切,可作为监测RRMS病情进展的指标。Objective To investigate the expression of serum miR-326 and Th17/Treg cells in relapsing-remitting multiple sclerosis(RRMS)and their clinical significance.Methods A total of 192 patients with RRMS admitted to the First Hospital of Hebei North College from January 2020 to December 2022 were prospectively selected for the study,and as and the study group the patients were divided into 107 cases in the acute stage and 85 cases in the remission stage according to the stage of disease development,and 96 healthy subjects who underwent health examination at the same hospital during the same period were selected as the control group according to a 2∶1 ratio.The differences between miR-326 expression and Th17/Treg cell-related indexes in each group were compared,and the clinical value of miR-326 expression and Th17/Treg cell-related indexes in predicting the occurrence of RRMS was analyzed,as well as the correlation between miR-326 expression,Th17/Treg cell-related indexes and different stages.Results Patients in the study group had significantly higher miR-326,interleukin(IL)-17A,IL-23 levels and Th17 percentages and significantly lower IL-35,TGF-β levels and Treg percentages than those in the control group(P<0.05).The miR-326,IL-17A,IL-23 levels and Th17 percentages were significantly higher in patients in the acute phase than in the remission phase,and the IL-35,TGF-β levels and Treg percentages were all significantly lower than in the remission phase(P<0.05).Receiver operating characteristic(ROC)curve analysis showed that the area under the curve(AUC)values for miR-326,IL-17A,IL-23,IL-35,TGF-β levels Th17 percentage,and Treg percentage to predict the occurrence of RRMS were 0.892,0.888,0.872,0.855,0.805,0.865,and 0.857,had good predictive value(P<0.05).Pearson analysis showed that miR-326,IL-17A,IL-23 levels and Th17 percentages were positively correlated with different stages(P<0.05),and IL-35,TGF-β levels and Treg percentages were negatively correlated with different stages(P<0.05).Spearman analysis showed th
