摘 要:目的 探讨超声造影参量成像(Parametric Imaging,PI)联合时间-强度曲线(Time-Intensity Curve,TIC)在颈部良恶性淋巴结疾病中的诊断价值。方法 回顾性分析我院经穿刺活检确诊的120例颈部淋巴结疾病患者的临床资料,术前均行超声造影PI及TIC分析,根据病理评价良恶性淋巴结特征差异。采用受试者工作特征(Receiver Operating Characteristic,ROC)曲线评价两种方法联合应用的诊断效能。结果 PI分析良性组淋巴结多为离心型灌注、增强均匀,而恶性组淋巴结多为向心型灌注、呈不均匀增强,缺损区边界不清,组间差异有统计学意义(P<0.05);TIC分析恶性组淋巴结与良性组相比,达峰时间较长、下降斜率较小,组间差异有统计学意义(P<0.05)。超声造影PI与TIC联合应用鉴别淋巴结良恶性的敏感度、特异性、ROC曲线下面积分别为91.0%,83.0%和0.870。结论 超声造影PI联合TIC有助于提高淋巴结良恶性鉴别诊断的敏感度和特异性。Objective To explore the diagnostic value of contrast-enhanced ultrasound parametric imaging(PI)combined with timeintensity curve(TIC)in benign and malignant cervical lymph node diseases.Methods The clinical data of 120 patients with cervical lymph node diseases diagnosed by puncture biopsy in our hospital were analyzed retrospectively.All of them were examined by contrast-enhanced ultrasound PI and TIC before biopsy.The evaluation of differences in the characteristics of benign and malignant lymph nodes was based on pathological results.The diagnostic efficacy of two methods individually and in combination was evaluated by using receiver operating characteristic(ROC)curve.Results PI analysis showed that the benign lymph nodes were mostly centrifugal perfusion and enhancement uniformity,while the malignant lymph nodes were mostly centripetal perfusion,uneven enhancement with unclear boundaries of the defect area,and the difference was statistically significant(P<0.05).The TIC analysis showed that the lymph nodes in the malignant group had a longer peak time and a slower slope of decline than those in the benign group,and the difference was statistically significant(P<0.05).The sensitivity of PI combined with TIC in distinguishing benign and malignant lymph nodes was 91.0%,the specificity was 83.0%and area under the ROC curve was 0.870,respectively.Conclusion Contrast-enhanced ultrasound PI combined with TIC can improve the sensitivity and specificity in the differential diagnosis of benign and malignant lymph nodes.
