真性小眼球白内障术后视网膜和脉络膜厚度变化

摘　　要：目的探讨真性小眼球病人白内障手术后黄斑中心凹视网膜厚度(CFT)及中心凹下脉络膜厚度(SFCT)的变化及其与病人术后视力的相关性。方法收集行超声乳化白内障吸除联合人工晶体植入术的真性小眼球病人37例52眼(真性小眼球组),以及正常眼轴病人40例52眼(正常眼轴组),比较两组病人术前及术后1周、1月、3月、6月CFT、SFCT的差异,Spearman相关分析法分析真性小眼球病人术后最佳矫正视力(BCVA)与CFT、SFCT的相关性。结果时间因素对于真性小眼球组和正常眼轴组CFT、SFCT的效应差异有统计学意义(F=5.251~104.101,P<0.05)。真性小眼球病人术后各时间段BCVA与其对应时间段CFT均呈负相关(r=-0.523~-0.423,P<0.05),术后各时间段BCVA与其对应时间段SFCT均无明显相关性(P>0.05)。结论白内障超声乳化吸除联合人工晶体植入手术引起真性小眼球病人眼底CFT、SFCT显著增厚,相比正常眼轴病人持续时间更长。Objective To investigate the changes in central foveal thickness(CFT)and subfoveal choroidal thickness(SFCT)after cataract surgery in patients with nanophthalmos,as well as their correlation with visual acuity after surgery.Me-thods A total of 37 patients(52 eyes)with nanophthalmos who underwent phacoemulsification combined with intraocular lens implantation were enrolled as nanophthalmos group,and 40 patients(52 eyes)with normal ocular axial length were enrolled as normal ocular axial length group.The two groups were compared in terms of the differences in CFT and SFCT before surgery and at 1 week,1 month,3 months,and 6 months after surgery,and a Spearman correlation analysis was used to investigate the correlation of best corrected visual acuity(BCVA)with CFT and SFCT after surgery in the patients with nanophthalmos.Results There was a significant difference in the effect of time factor on CFT and SFCT between the nanophthalmos group and the normal ocular axial lengthgroup(F=5.251-104.101,P<0.05).For the patients with nanophthalmos,there was a negative correlation between BCVA and CFT in each time period after surgery(r=-0.523 to-0.423,P<0.05),but there was no significant correlation between BCVA and SFCT(P>0.05).Conclusion Cataract phacoemulsification combined with intraocular lens implantation can cause significant thickening of CFT and SFCT in patients with nanophthalmos,which lastslonger compared with thepatients with normal ocular axial length.
