摘　　要：目的分析潍坊地区幽门螺杆菌(Hp)感染者胃黏膜Hp培养的阳性率及其影响因素,获取潍坊地区Hp药敏谱,为本地区治疗Hp感染抗生素的选择提供指导。方法回顾性分析2019年1月~2019年12月行胃镜检查或^(13)C/^(14)C呼气试验确诊Hp感染患者412例,利用平皿掺入法分离培养Hp,琼脂稀释法检测Hp对克拉霉素、阿莫西林、左氧氟沙星、甲硝唑、呋喃唑酮、四环素的敏感性,获取Hp培养阳性率及药敏谱。结果Hp培养阳性率随患者年龄增加呈现上升趋势(P<0.05),上消化道溃疡患者Hp培养阳性率明显高于炎症性疾病患者(P<0.05)。耐药性方面,Hp对甲硝唑的耐药率最高,且易发生双重及多重耐药;对阿莫西林的耐药率最低;对克拉霉素、左氧氟沙星的耐药率随年龄增加呈升高趋势;各种药物耐药率与性别无明显相关性。结论潍坊地区Hp感染率较高,且对甲硝唑严重耐药,对阿莫西林耐药率极低,因此推荐阿莫西林作为本地区治疗胃黏膜感染Hp的首选抗菌药物。Objective To analyze the positive rate of Helicobacter pylori(Hp)infection in the gastric mucosa of patients in Weifang area and its influencing factors,to obtain the Hp drug sensitivity spectrum,to provide guidance for the selection of antibiotics for treating patients with Hp infection.Methods Retrospective analysis was made on 412 patients with Hp infection diagnosed by gastroscopy or^(13)C/^(14)C breath test from January 2019 to December 2019.Hp was isolated and cultured by plate incorporation method,and the sensitivity of Hp to Clarithromycin,Amoxicillin,Levofloxacin,Metronidazole,Furazolidone,and Tetracycline was detected by agar dilution method to obtain the positive rate of Hp culture and drug sensitivity spectrum.Results The positive rate of Hp culture showed an increasing trend with the age of patients(P<0.05);and the positive rate of Hp culture in patients with upper gastrointestinal ulcers was significantly higher than that in patients with inflammatory diseases(P<0.05);Hp has the highest resistance rate to Metronidazole,and is prone to double and multiple drug resistance;The drug resistance rate to Amoxicillin was the lowest;The resistance rate to Clarithromycin and Levofloxacin increased with age;There is no significant correlation between drug resistance rates and gender.Conclusion The infection rate of Hp in Weifang area is high and the drug resistance to Metronidazole is serious,and the drug resistance rate to Amoxicillin is extremely low.Therefore,Amoxicillin is recommended as the first choice of antibiotics for treating Hp infection in Gastric mucosa.
