胃癌组织凋亡抑制基因Survivin和MMP-2蛋白表达及其与Hp感染的关系

摘　　要：目的探讨胃癌组织凋亡抑制基因存活素(Survivin)和基质金属蛋白酶2(MMP-2)蛋白表达及其与幽门螺杆菌(Hp)感染的关系.方法选取2019年6月-2022年6月海口市第四人民医院收治的65例胃癌患者为胃癌组,139例胃癌前病变患者为胃癌前病变组,138例慢性胃炎患者为对照组.取患者胃癌组织标本或胃黏膜组织标本,检测Survivin、MMP-2蛋白表达及Hp感染情况.统计胃癌患者不同临床特征和Hp感染Survivin、MMP-2蛋白表达情况,采用Spearman相关系数法分析Survivin、MMP-2蛋白与Hp感染的相关性.结果胃癌组、胃癌前病变组、对照组Survivin、MMP-2蛋白阳性表达率呈逐渐降低趋势(P<0.05);胃癌组、胃癌前病变组Hp阳性率高于对照组(P<0.05);TNM分期为Ⅲ、Ⅳ期,分化程度为低分化型,有淋巴结转移患者胃癌组织Sur-vivin、MMP-2蛋白阳性表达率较高(P<0.05);Hp阳性组胃癌组织凋亡抑制基因Survivin、MMP-2蛋白阳性表达率分别为79.07%、84.21%,均高于Hp阴性组(P<0.05);Spearman相关性分析结果显示,凋亡抑制基因Survivin、MMP-2蛋白与Hp感染呈正相关(P<0.05).结论胃癌组织凋亡抑制基因Survivin和MMP-2蛋白均呈高表达,且表达均与Hp感染及胃黏膜癌变发生、发展密切相关.OBJECTIVE To explore the expressions of apoptosis suppressor gene Survivin and matrix metalloproteinase-2(MMP-2)in gastric cancer tissues and analyze their association with Helicobacter pylori infection.METHODS Totally 65 patients with gastric cancer who were treated in Haikou Fourth People's Hospital from Jun 2019 to Jun 2022 were assigned as the gastric cancer group,139 patients who had gastric precancerous lesions were assigned as the gastric precancerous lesion group,and 138 patients with chronic gastritis were chosen as the control group.The gastric cancer tissues or gastric mucosa tissue specimens were collected from the patients;the expression levels of Survivin and MMP-2 were detected,and the prevalence of Hp infection was analyzed.RESULTS The positive rates of expressions of Survivin and MMP-2 protein showed downward trends in the gastric cancer group,the gastric precancerous lesion group and the control group(P<0.05).The positive rate of Hp was higher in the gastric cancer group and the gastric precancerous lesion group than in the control group(P<0.05).The positive rates of expressions of Survivin and MMP-2 protein in gastric cancer tissues were relatively high among the patients with TNM stage of III-IV,low differentiation type,and lymphatic metastasis(P<0.05).The positive rates of expressions of apoptosis suppressor gene Survivin and MMP-2 protein in gastric cancer tissues were respectively 79.07%and 84.21%in the Hp-positive group,higher than those of the Hp-negative group(P<0.05).The Spearman correlation analysis showed that the apoptosis suppressor gene Survivin and MMP-2 protein were positively correlated wit the Hp infection(P<O.05).CONCLUSION The apoptosis suppressor gene Survivin and MMP-2 protein are expressed highly in gastric cancer tissues,which is closely associated with the Hp infection,occurrence and development of carcinogenesis of gastric mucosa.
