摘  要:目的探讨EB病毒(EBV)感染对慢性牙周炎(CP)患者Toll样受体(TLRs)信号通路的影响,为临床治疗提供客观依据.方法选取2018年11月-2021年5月于河北省眼科医院治疗的106例慢性牙周炎患者为研究对象,根据有无EBV感染分为EBV感染组35例和非EBV感染组71例,比较两组牙龈指数、附着丧失程度,聚合酶链式反应(PCR)检测TLRs信号通路相关因子、转化生长因子激酶1(TAK1)和TANK结合激酶1(TBK1)mRNA相对表达量,常规检查白细胞计数(WBC)、淋巴细胞比例、B细胞比例、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平.结果两组牙龈指数和附着丧失程度比较差异无统计学意义;EBV感染组TLR2、TLR4、TLR7、TAK1、TBK1 mRNA相对表达量均高于非EBV感染组(P<0.05),WBC、淋巴细胞比例、B细胞比例、IL-6、TNF-α水平均高于非EBV感染组(P<0.05).结论CP患者在EBV感染后,刺激TLRs信号通路过度活化,进一步导致炎性反应失衡,可能是引起CP患者免疫功能紊乱的重要原因之一,但EBV感染与牙周病变程度无关.OBJECTIVE To explore the influence of Epstein-Barr virus(EBV)infection on Toll-like receptors(TLRs)signaling pathways of the patients with chronic periodontitis(CP)so as to provide objective basis for clinical treatment.METHODS A total of 106 CP patients who were treated in Hebei Eye Hospital from Nov 2018 to May 2021 were recruited as research subjects and were divided into the EBV infection group with 35 cases and the non-EBV infection group with 71 cases according to the status of EBV infection.The gingival indexes and degree of attachment loss were compared between the two groups.The expression levels of TLRs signaling pathways,transforming growth factor kinase 1(TAK1)and TANK-binding kinase 1(TBK1)mRNA were detected by means of polymerase chain reaction(PCR).The routine test of white blood cell(WBC),percentage of lymphocyte,percentage of B cell,interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)was carried out.RESULTS There were no significant differences in the gingival indexes and degree of attachment between the two groups.The relative expression levels of TLR2,TLR4,TLR7,TAKl and TBK1 mRNA of the EBV infection group were higher than those of the non-EBV infection group(P<0.05);the WBC level,percentage of lymphocyte,percentage of B cell,IL-6 level and TNF-αlevel were higher in the EBV infection group than in the non-EBV infection group(P<0.05).CONCLUSION The EBV infection may stimulate the excessive activation of TLRs signaling pathways of the CP patients and then further result in the imbalance of inflammatory response,which is one of the leading causes of immune dysfunction,however,the EBV infection is not associated with the degree of periodontal disease.
