P53基因单核苷酸多态性及HPV16/18感染与食管癌的关联

摘　　要：目的分析P53基因单核苷酸多态性及人乳头瘤病毒(HPV)16/18感染与食管癌的关系。方法选取2018年5月-2021年5月重庆两江新区人民医院收治的103例食管癌患者(食管癌组)和100例食管黏膜炎病变患者(对照组),采用聚合酶链式反应-限制性片段长度多态性(PCR-RFLP)法检测P53基因单核苷酸多态性,采用PCR检测HPV16/18亚型分布情况。结果两组P53基因rs1042522位点Pro/Pro、Pro/Arg、Arg/Arg基因型及Pro、Arg等位基因分布频率比较,差异均有统计学意义(P<0.05);食管癌患者P53基因rs1042522位点多态性与分化程度、TNM分期和淋巴结转移有关(P<0.05);食管癌组组织中HPV16、HPV18阳性率分别为52.43%、32.04%,高于对照组的16.00%、10.00%(P<0.05);食管癌组HPV16阳性、HPV18阳性患者P53基因rs1042522位点Arg/Arg基因型占比均高于HPV16阴性、HPV18阴性患者(P<0.05)。结论P53基因rs1042522位点多态性可能是食管癌遗传易感性的因素之一,Arg/Arg基因型携带者更容易发生HPV16/18相关食管癌。OBJECTIVE To observe the relationship between single nucleotide polymorphisms(SNPs)of P53 gene,human papillomavirus(HPV)16/18 infections and esophageal cancer.METHODS Totally 103 patients with esophageal cancer(the esophageal cancer group)and 1oo patients with esophageal mucositis lesions(the control group)who were treated in the People's Hospital of Chongqing Liangjiang New Area from May 2018 to May 2021 were enrolled in the study.The SNPs of P53 gene were detected by means of polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP),and the HPV16/18 subtypes were detected by PCR.RESULTS There were significant differences in the frequencies of Pro/Pro,Pro/Arg,Arg/Arg genotypes and Pro,Arg alleles at rs1042522 locus of P53 gene between the two groups(P<0.05).The polymorphisms at rs1042522 locus of P53 gene were associated with the degree of differentiation,TNM stage and lymph node metastasis(P<0.05).The positive rate of HPV16 of the esophageal cancer group was 52.43%,higher than 16.00%of the control group;the positive rate of HPV18 of the esophageal cancer group was 32.04%,higher than 10.00%of the control group(P<0.05).Among the esophageal cancer patients,the proportions of the HPV16-and HPV18-psitive patients who carried with Arg/Arg genotype at rs1042522 locus of P53 gene were higher than those of the HPV16-and HPV18-negative patients(P<0.05).CONCLUSION The polymorphism at rs1042522 locus of P53 gene may be one of the factors for genetic susceptibility of esophageal cancer,and the patients who carry with Arg/Arg genotype are more likely to have HPV16/HPV18-related esophageal cancer.
