微创玻璃体切割术联合翻转覆盖治疗大孔径特发性黄斑裂孔的疗效观察

摘　　要：目的分析微创玻璃体切割术联合翻转覆盖治疗大孔径特发性黄斑裂孔(LIMH)的疗效。方法选取福州东南眼科医院2017年6月—2018年6月收治的80例(80眼)LIMH患者作为研究对象,根据治疗方法分为研究组和对照组,每组40例(40眼)。所有患者均接受25G切口微创玻璃体切割术,研究组在此基础上增加翻转覆盖术。观察2组手术前后最佳矫正视力(BCVA)、眼压、多焦视网膜电图(mfERG)一阶函数的1~2环P1波反应密度和并发症,对比2组术后裂孔闭合率。结果手术6个月后,2组BCVA和、眼压升高,且研究组BCVA和眼压高于对照组(P<0.05)。研究组术后各时间点的裂孔闭合率均高于对照组(P<0.05)。手术6个月后,2组mfERG一阶函数的1~2环P1波反应密度均升高,且研究组高于对照组(P<0.05)。2组均未出现严重并发症。结论微创玻璃体切割术联合翻转覆盖术能够改善LIMH患者视功能,促进黄斑裂孔闭合,安全性高,值得推广。Objective To analyze the effect of minimally invasive vitrectomy combined with flip cover for large aperture idiopathic macular hole(LIMH).Methods A total of 80 patients(80 eyes)with LIMH who were admitted to Fuzhou Southeast Ophthalmic Hospital from June 2017 to June 2018 were included in the study.According to the differences in treatment methods,they were divided into a study group and a control group,with 40 cases(40 eyes)in each group.All the patients received minimally invasive vitrectomy with a 25-gauge incision,and the research group added flip cover on top of this.The best corrected visual acuity(BCVA),intraocular pressure(IOP),1-2 ring P1 wave response density of first-order function of muhifocalelectroretinogram(GmfERG)and complications were observed before and after operation in the two groups,and the closure rate of hiatus between the two groups was compared.Results After 6 months of operation,the BCVA and IOP of the two groups increased,and the BCVA and IOP of the study group was higher than those of the control group(P<0.05).The rates of hiatus closure at each time point after surgery in the study group were higher than those in the control group(P<0.05).Six months after surgery,the P1 wave reaction densities of the 1-2 rings of the first-order function of mfERG in the two groups were increased,and those in the research group were higher than those in the control group(P<0.05).No serious complications occurred in the two groups.Conclusion Minimally invasive vitrectomy combined with flip cover surgery can improve the visual function of patients with LIMH and promote the closure of macular hole.With high safety,it is worthy of promotion.
