摘  要:目的 探讨CT血管成像(CTA)和三维重建技术在股前外侧皮瓣移植修复口腔颌面部大面积组织缺损中的应用效果。方法 回顾性分析2018年3月至2023年1月在我院接受股前外侧皮瓣移植修复口腔颌面部大面积组织缺损患者临床资料,根据术前对穿支血管检查方法不同,将其分为CT组和CDU (彩色多普勒超声)组,CT组术前采用CTA和三维重建技术对股外侧动脉及穿支血管定位和评估,CDU组采用彩色多普勒超声进行定位和评估,比较两组患者临床资料、术前和术中穿支动脉内径和穿支血管皮肤浅出点位置差值、手术时间、术中出血量、皮瓣存活率、术后并发症发生率和美容满意度。结果 共收集到35例患者,其中CT组18例,CDU组17例,两组一般资料比较差异均无统计学意义(P>0.05)。CT组术前对穿支动脉内径和穿支血管皮肤浅出点测定准确度均高于CDU组,差异具有统计学意义(P<0.05)。CT组手术时间较CDU组短、术中出血量较CDU组少、皮瓣存活率较CDU组高、术后并发症发生率较CDU组低,美容满意度较CDU组高,差异均有统计学意义(P<0.05)。结论 CT血管成像和三维重建技术对移植皮瓣穿支血管的评估和定位较CDU精确,能够为手术提供更为精确的临床数据,是保障手术顺利完成、降低患者损伤和术后并发症发生率、提升皮瓣移植存活率和美容效果的有效手段。Objective Explore the application effffect of CT angiography(CTA)and three-dimensional reconstruction technology in the transplantation of anterolateral femoral skin flflap for repairing large tissue defects in the oral and maxillofacial regions.Methods Retrospective analysis of clinical data of patients who underwent anterolateral femoral skin flflap transplantation to repair large tissue defects in the oral and maxillofacial regions in our hospital from March 2018 to January 2023.According to difffferent preoperative examination methods for perforating vessels,they were divided into a CT group and a CDU(color Doppler ultrasound)group.The CT group used CTA and three-dimensional reconstruction technology to locate and evaluate the lateral femoral artery and perforating vessels before surgery,while the CDU group used color Doppler ultrasound for localization and evaluation,Compare the clinical data,preoperative and intraoperative difffferences in the diameter of the perforating artery and the superfificial point position of the perforating vessel skin between the two groups of patients,surgical time,intraoperative bleeding,flflap survival rate,postoperative complication rate,and cosmetic satisfaction.Results A total of 35 patients were collected,including 18 in the CT group and 17 in the CDU group.There was no statistically signifificant difffference in general data between the two groups(P>0.05).The accuracy of preoperative measurement of the diameter of the perforating artery and the superfificial point of the perforating vessel skin in the CT group was higher than that in the CDU group,and the difffference was statistically signifificant(P<0.05).The CT group had shorter surgical time,less intraoperative bleeding,higher flflap survival rate,lower postoperative complications,and higher cosmetic satisfaction compared to the CDU group,with statistically signifificant difffferences(P<0.05).Conclusion CT vascular imaging and three-dimensional reconstruction technology are more accurate in evaluating and locating the
