骨皮质切开术上颌快速扩弓辅助治疗成人骨性Ⅲ类错[牙合]畸形患者的效果

摘　　要：目的探讨骨皮质切开术上颌快速扩弓辅助治疗成人骨性Ⅲ类错[牙合]畸形患者的效果。方法选取2020年8月-2022年8月本院收治的成人骨性Ⅲ类错[牙合]畸形患者92例,随机将患者分为常规组和治疗组,每组各46例。常规组患者采用正颌手术治疗,治疗组在常规组的基础上采用骨皮质切开术上颌快速扩弓辅助治疗。治疗后,比较两组患者的牙排齐时间、牙间隙关闭时间,在治疗前后,比较两组患者的牙周临床指标[菌斑指数(PLI)、出血指数(BI)、角化龈宽度(WKG)],上中切牙根尖至冠状面的距离(U1I-CP)、鼻小柱顶端一鼻底与鼻底一上唇红唇之间的交角(NLA)、唇上点与冠状面之间的垂直距离(Ls-CP),治疗后,比较两组患者的不良反应发生情况。结果治疗后,治疗组患者牙排齐时间、牙间隙关闭时间均短于常规组患者,差异具有统计学意义(P<0.05);与治疗前比较,治疗后两个月两组患者PLI、BI指数均降低,WKG升高,差异具有统计学意义(P<0.05),治疗后两个月,治疗组患者PLI、BI指数低于常规组,WKG高于常规组,差异具有统计学意义(P<0.05);与治疗前比较,治疗后两个月两组患者U1I-CP、NLA、Ls-CP均升高,差异具有统计学意义(P<0.05),治疗后两个月,治疗组患者U1I-CP、NLA、Ls-CP均高于常规组,差异具有统计学意义(P<0.05);治疗后,常规组患者出现1例咀嚼困难,治疗组患者未出现不良反应,两组患者不良反应发生情况比较无显著差异(P>0.05)。结论骨皮质切开术上颌快速扩弓辅助治疗成人骨性Ⅲ类错[牙合]畸形能够加速牙移动,减少患者术后牙周菌斑和牙周出血,促进患牙恢复。Objective To investigate the effffect of osteotomy maxillary rapid arch extension in adult patients with osteoid error(occlusion)deformity.Methods 92 adult patients with bone occlusion(occlusion)admitted to our hospital from August 2020 to August 2022 were selected,and randomly divided into conventional group and treatment group with 46 patients in each group.Patients in the conventional group were treated with orthognathic surgery,and the treatment group was assisted by osteocorticotomy on the basis of the conventional group.After treatment,compare the tooth alignment time and closing time of the two groups,compare the periodontal clinical index[plaque index(PLI),bleeding index(BI)(WKG)],the distance between the tip from the root to the coronal plane(U1I-CP),the angle between the upper lip(NLA),the vertical distance between the upper lip and the coronal plane(Ls-CP).After treatment,the adverse reactions of the two groups were compared.Results After the treatment,The dental alignment time and dental space closure time in the treatment group were shorter than those in the conventional group,The difffference was statistically signifificant(P<0.05);In comparison with the pre-treatment period,PLI and BI index decreased in both groups two months after treatment,WKG go up,The difffference was statistically signifificant(P<0.05),Two months after the treatment,The PLI and BI index were lower than those in the conventional group,The WKG was higher than that in the conventional group,The difffference was statistically signifificant(P<0.05);In comparison with the pre-treatment period,U1I-CP,NLA and Ls-CP were increased in two groups two months after treatment,The difffference was statistically signifificant(P<0.05),Two months after the treatment,U1I-CP,NLA,CP and Ls-CP than those in the conventional group,The difffference was statistically signifificant(P<0.05);post-treatment,The patient in the conventional group had one diffifficulty in chewing,No adverse effffects were observed in the treatment group,There was no signifif
