血清CA242、CA199、CA724、CEA检测对胃癌诊断、预后评估及与免疫反应指标相关性研究

摘　　要：目的探讨血清糖类抗原242(CA242)、糖类抗原199(CA199)、糖类抗原724(CA724)和癌胚抗原(CEA)检测在胃癌诊断中的应用。方法选取在南阳市中心医院治疗的60例胃癌患者(胃癌组),60例胃良性病变患者(胃良性疾病组)为研究对象,另选同一时期40例在本院体检无异常者为对照组,分别检测和比较各组CA242、CA199、CA724、CEA和CD4^(+)CD25^(+)T淋巴细胞。结果胃癌组血清CA242、CA199、CA724和CEA水平均显著高于对照组和胃良性疾病组(P<0.05);胃癌组中Ⅲ期+Ⅳ期、低中分化、肿瘤转移患者血清CA724、CA199、CA242、CEA平均水平均高于Ⅰ期+Ⅱ期、高分化和肿瘤未转移患者,差异均有统计学意义(P<0.05);四项指标单项检测灵敏度CA199>CEA>CA724>CA242;CA199、CEA、CA724、CA242和联合检测诊断胃癌的AUC分别为0.791、0.759、0.740、0.672和0.943;胃癌患者血清CA242、CA199、CA724、CEA与CD4^(+)CD25^(+)T淋巴细胞呈正相关(r_(CA199)=0.6141,r_(CA724)=0.6214,r_(CA242)=0.5806,r_(CEA)=0.5568,P<0.001)。结论胃癌患者血清CA242、CA199、CA724和CEA水平异常升高,四项指标联检诊断胃癌的敏感度和诊断效能均高于单一指标检测。血清CA242、CA199、CA724和CEA与外周血CD4^(+)CD25^(+)T淋巴细胞呈正相关,监测血清CA242、CA199、CA724和CEA可评价胃癌患者预后。Objective To explore the application of CA242,CA199,CA724 and CEA in the diagnosis of gastric cancer.Methods 60 patients with gastric cancer in our hospital were selected as the gastric cancer groep,60 patients with benign disease as the benign disease groep,another 40 patients with normal healthy examination were selected as the contral group,detecting CA242,CA199,CA724,CEA and CD4+CD25+T lymphocyte of each groups.Results The level of CA242,CA199,CA724,CEA in he gastric cancer groep were significantly higher than those in control group and benign disease groep(P<0.05);The level of serum CA242,CA199,CA724 and CEA inⅠstage andⅡstage,low and moderately degree of differentiation,and metastasis of gastric cancer patients were significantly higher compared withⅠstage andⅡstage,high differentiation,and no metastasis of gastric cancer patients(P<0.05).The sensitivity of single detection was CA199>CEA>CA724>CA242,The area under ROC curve of CA199,CEA,CA724,CA242 and combined detection in gastric cancer were 0.791,0.759,0.740,0.672 and 0.943.The level of CA242,CA199,CA724 and CEA in gastric cancer patients were positively correlated with CD4^(+)CD25^(+)T lymphocyte(r_(CA199)=0.6141,r_(CA724)=0.6214,r_(CA242)=0.5806,r_(CEA)=0.5568,P<0.001).Conclusion Serum CA242,CA199,CA724 and CEA level of gastric cancer patients were exceptional higher,the combined detection of serum CA242,CA199,CA724 and CEA can improve the sensitivity and diacrisis efficacy of gastric cancer diagnosis.The level of CA242,CA199,CA724 and CEA in gastric cancer patients were positively correlated with CD4^(+)CD25^(+)T lymphocyte,monitoring serum CA242,CA199,CA724 and CEA can evaluate prognosis condition of gastric cancer patients.
