摘　　要：目的 探讨2型糖尿病与帕金森患者运动迟缓和姿势平衡障碍的相关性。方法 纳入2021年1月-2022年6月就诊于新疆维吾尔自治区人民医院神经内科的帕金森病(Parkinson′s disease, PD)患者138例,按照是否伴有糖尿病将其分为PD-DM组和对照组。所有入组人员完善空腹血糖、糖化血红蛋白、UPDRS评分、H-Y分级的评估,比较两组之间的差异。在此基础上将UPDRS第Ⅲ部分进一步分为震颤、强直、姿势平衡障碍以及运动迟缓四个小项目,分析两组在不同项目之间的差异。结果 PD-DM组的空腹血糖、糖化血红蛋白水平高于对照组(P<0.001),两组在UPDRS评分和H-Y分级没有差异(P>0.05)。另外,相对于对照组,PD-DM组患者在运动迟缓和姿势平衡障碍方面的危险性增加(P<0.001),且在运动迟缓方面更明显(β=3.581vs.β=2.743),但对震颤和僵直没有影响(P>0.05)。结论 糖尿病可能影响PD患者的病情进展及运动功能,主要的运动症状体现在运动迟缓及姿势平衡障碍方面。Objective To investigate relationship between type 2 diabetes and motor retardation and postural balance disorder of Parkinson's patients.Methods The paper included 138 PD patients in neurology department of Xinjiang Uygur Autonomous Region People's Hospital from January 2021 to June 2022,and divided them into PD-DM group and control group according to whether there was diabetes.All participants were evaluated with fasting blood sugar,glycated hemoglobin,UPDRS scores and H-Y grade,difference between two groups was compared.On the basis,UPDRS PartⅢwas divided into four sub items:tremor,rigidity,postural balance disorders and motor delay,difference between two groups in different items was analyzed.Results Levels of fasting blood sugar and glycated hemoglobin in PD-DM group were higher than control group(P<0.05).In addition,PDDM group had higher risk of motor delay and postural balance disorders than control group(P<0.001),motor delay was more significatn,(β=3.581 vs.,β=2.743),with no effect on tremor and stiffness(P>0.05).Conclusion Diabetes may affect disease progression and motor function of PD patients,with main motor symptoms of motor retardation and postural balance disorder.
