摘　　要：目的 比较不同术式在治疗急性闭角型青光眼合并白内障中的疗效。方法 将2022年2-11月在本院眼科治疗的39例46眼急性闭角型青光眼合并白内障患者随机分为两组,每组23眼。对照组使用超声乳化及人工晶体植入联合小梁切除术治疗,观察组使用超声乳化及人工晶体植入联合房角分离术治疗,对比两组的临床疗效、最佳矫正视力、眼压、并发症。结果 观察组治疗有效率为95.65%,明显高于对照组的65.22%(P<0.05);两组术后1周、1个月、3个月的最佳矫正视力、眼压相比,观察组视力逐渐升高并稳定,且均高于对照组,而眼压明显降低,且均低于对照组(P<0.05);两组浅前房、前房积血、角膜水肿、黄斑水肿等并发症情况相比,观察组发生率明显低于对照组(P<0.05)。结论 超声乳化及人工晶体植入联合房角分离术治疗急性闭角型青光眼合并白内障的效果更好,能有效提升视力,降低并稳定眼压,且其并发症发生率低。Objective To compare curative effect of different surgical methods for acute angleclosure glaucoma with cataract.Methods The paper chose 39 patients(46 eyes)with acute angle-closure glaucoma and cataract in ophthalmology department of our hospital from February 2022 to November 2022,and divided them into two groups randomly,with 23 eyes in each group.Control group was treated with phacoemulsification and intraocular lens implantation combined with trabeculectomy,while observation group with phacoemulsification and intraocular lens implantation combined with angle separation surgery.Clinical effect,optimal corrected vision,intraocular pressure and complications between two groups were compared.Results Effective rate of treatment in observation group was 95.65%,significantly higher than 65.22%in control group(P<0.05).Compared with the best corrected visual acuity and intraocular pressure at 1 week,1 month and 3 months after surgery,the visual acuity in observation group gradually increased and stabilized,higher than control group,while intraocular pressure significantly decreased,lower than control group(P<0.05).Incidence of complications such as shallow anterior chamber,hyphema,corneal edema and macular edema in observation group was significantly lower than control group(P<0.05).Conclusion Combination of phacoemulsification and intraocular lens implantation with angle separation surgery can achieve good curative effect on acute angle-closure glaucoma with cataract,can improve vision,reduce and stabilize intraocular pressure effectively,which has low incidence of complications.
