摘　　要：目的探讨C⁃erbB⁃2和C⁃MYC基因在非小细胞肺癌(NSCLC)中的表达及其与预后的关系。方法选取2016年1月至2018年3月河西学院附属张掖人民医院89例NSCLC患者,所有患者均行肺癌根治术,术中切除癌组织以及距病灶3 cm的癌旁组织制作石蜡切片。采用免疫组化技术检测NSCLC癌组织以及癌旁组织C⁃erbB⁃2及C⁃MYC表达情况。对所有患者进行5年随访,分析生存情况。结果NSCLC癌组织的C⁃erbB⁃2阳性表达率为43.82%,高于癌旁组织的7.87%(χ^(2)=30.018,P<0.001)。NSCLC癌组织的C⁃MYC阳性表达率为61.80%,高于癌旁组织的17.98%(χ^(2)=35.637,P<0.001)。C⁃erbB⁃2阳性患者的总生存期为10.6~60.0个月,5年生存率为38.46%;C⁃erbB⁃2阴性患者的总生存期为13.4~60.0个月,5年生存率为58.00%;两组总生存期比较差异有统计学意义(Log rank=4.086,P=0.043)。C⁃MYC阳性患者的总生存期为10.6~60.0个月,5年生存率为38.18%;C⁃MYC阴性患者的总生存期为15.5~60.0个月,5年生存率为67.65%;两组总生存期比较差异有统计学意义(Log rank=8.141,P=0.004)。结论C⁃erbB⁃2和C⁃MYC表达情况与NSCLC患者预后密切相关,可作为分子标志物评估NSCLC患者预后。Objective To investigate the expression of C⁃erbB⁃2 and C⁃MYC in non⁃small cell lung cancer(NSCLC),and their relationship with the prognosis.Methods The clinical data of 89 patients with NSCLC treated in the Zhangye People's Hospital Affiliated to Hexi University between January 2016 and March 2018 were analyzed retrospectively.All patients underwent radical operation of lung cancer.Cancer tis⁃sues removed during the operation,and tissues 3 cm away from the lesion were collected.The expression of C⁃erbB⁃2 and C⁃MYC in NSCLC tissues and adjacent tissues was detected with immunohistochemical tech⁃niques.All patients were followed up for 5 years,and their survival status was analyzed.Results The posi⁃tive expression rates of C⁃erbB⁃2 and C⁃MYC in NSCLC tissues(43.82%and 61.80%)were higher than those in adjacent tissues(7.87%and 17.98%).(χ^(2)=30.018,P<0.001;χ^(2)=35.637,P<0.001).The proportions of pa⁃tients with adenocarcinoma and patients with poor differentiation among C⁃erbB⁃2 positive patients were higher than those in C⁃erbB⁃2 negative patients.(χ^(2)=5.751,P=0.016;χ^(2)=9.191,P=0.010).The overall survival time of C⁃erbB⁃2 positive patients was 10.6⁃60.0 months,and the 5⁃year survival rate was 38.46%.The overall survival time of C⁃erbB⁃2 negative patients was 13.4⁃60.0 months,and the 5⁃year survival rate was 58.00%.There was a statistically significant difference in overall survival time between the two groups(Log rank=4.086,P=0.043).The overall survival time of C⁃MYC positive patients was 10.6⁃60.0 months,and the 5⁃year survival rate was 38.18%.The overall survival time of C⁃MYC negative patients was 15.5⁃60.0 months,and the 5⁃year survival rate was 67.65%.There was a statistically significant difference in overall survival time between the two groups(Log rank=8.141,P=0.004).Conclusion The expression of C⁃erbB⁃2 and C⁃MYC is related to the prognosis of NSCLC and can be used as molecular markers to evaluate prognosis.
