末端回肠造瘘术和横结肠造瘘术治疗腹腔镜下直肠癌根治术后吻合口瘘的效果分析

摘　　要：目的:分析末端回肠造瘘术和横结肠造瘘术治疗腹腔镜下直肠癌根治术后吻合口瘘的效果。方法:选取2017年11月—2021年12月于常州市中医医院行直肠癌根治术且术后发生吻合口瘘的患者30例作为研究对象,采用随机数字表法分为横结肠造瘘组与末端回肠造瘘组,各15例。横结肠造瘘组接受横结肠造瘘术治疗,末端回肠造瘘组接受末端回肠造瘘术治疗。比较两组吻合口瘘处理效果、生活质量及满意度。结果:两组吻合口瘘治愈率比较,差异无统计学意义(P>0.05)。治疗前,两组生理功能、身体疼痛、活力、健康状况、情感职能、精神健康、生理职能、社会功能评分比较,差异无统计学意义(P>0.05);治疗后,末端回肠造瘘组生理功能、身体疼痛、活力、健康状况、情感职能、精神健康、生理职能、社会功能评分高于横结肠造瘘组,差异有统计学意义(P<0.05)。两组满意率比较,差异无统计学意义(P>0.05)。结论:末端回肠造瘘术和横结肠造瘘术治疗腹腔镜下直肠癌根治术后吻合口瘘的效果相当,患者满意度均较高,但末端回肠造瘘术后患者生活质量更高。Objective:To analyze the effect of terminal ileostomy and transverse colostomy in treatment of anastomotic leakage after laparoscopic radical resection of rectal cancer.Methods:Thirty patients undergoing radical resection of rectal cancer and experiencing postoperative anastomotic leakage in Changzhou Hospital of Traditional Chinese Medicine from November 2017 to December 2021 were selected as the study subjects.According to random number table method,they were divided into transverse colostomy group and terminal ileostomy group,with 15 cases in each group.The transverse colostomy group received transverse colostomy.The terminal ileostomy group received terminal ileostomy.The treatment effect of anastomotic leakage,quality of life and satisfaction were compared between the two groups.Results:There was no significant difference in the cure rate of anastomotic leakage between the two groups(P>0.05).Before treatment,there was no significant difference in the scores of physiological function,physical pain,vitality,health status,emotional function,mental health,physiological function and social function between the two groups(P>0.05).After treatment,the scores of physiological function,physical pain,vitality,health status,emotional function,mental health,physiological function and social function in the terminal ileostomy group were higher than those in the transverse colostomy group,and the differences were statistically significant(P<0.05).There was no significant difference in satisfaction rate between the two groups(P>0.05).Conclusion:The effect of terminal ileostomy and transverse colostomy in the treatment of anastomotic leakage after laparoscopic radical resection of rectal cancer is comparable,and the satisfaction of patients is higher,but the quality of life of patients after terminal ileostomy is higher.
