缺陷型精神分裂症海马亚结构萎缩与注意功能障碍

摘　　要：目的:缺陷型精神分裂症(DS)被认为是精神分裂症谱系中的一个独特疾病实体。探讨DS的海马亚结构异常以及与这种精神分裂症亚型的独特症状的关联。方法:比较33例DS、39例非缺陷型精神分裂症(NDS)患者和38名健康受试者的海马亚区容积和神经心理学评估。通过阳性与阴性症状量表(PANSS)评估临床症状。通过成套神经认知量表(包括数字划销测试,动物命名测试,受控口语联想测试、跟踪测试和Stroop色词测试)进行认知测试;对患者海马亚结构体积进行分析。结果:与NDS患者相比,DS患者在左侧海马的5个亚区和右侧海马的9个亚区表现出明显的体积萎缩(P均<0.05)。此外,在DS患者中,右侧CA1_头部萎缩与注意力缺陷有关。结论:研究海马亚区域萎缩模式对于更深入地了解DS认知缺陷的病理生理学具有重要意义。Objective:Deficit schizophrenia(DS)is considered a distinct disease entity within the schizophrenia spectrum.To investigate the possibility of regional hippocampal abnormalities specific to DS and associations with the unique symptoms of this schizophrenia subtype.Method:This study compared 33 DS and 39 non-deficit schizophrenia(NDS)patients and 38 healthy subjects for hippocampal subfield volumetry and neuropsychological assessment such as digital marketing test,animal naming test,controlled oral word association test,eye tracking test and Stroop color and word test.Clinical symptoms and cognitive function were measured by positive and negative symptom scale(PANSS)and neurocognitive battery,respectively.The volume of hippocampal substructure was analyzed.Results:Compared to NDS patients,DS patients exhibited greater volume loss in 5 subregions of the left hippocampus and 9 subregions of the right hippocampus.Moreover,in DS patients,atrophy of right CA1_head was associated with attention deficits.Conclusion:Studying hippocampal subregional atrophy patterns has important implications for gaining a deeper understanding of the pathophysiology of cognitive deficits in DS.
