犬颌骨骨折小钛板坚强内固定方式的探讨

摘  要:目的:探讨小钛板用于颌骨骨折两种坚强内固定方式的固定效果。方法:选用24只杂种犬,随机分为A(上颌骨对照组)、B(下颌骨对照组)、C(上颌骨实验组)和D(下颌骨实验组)4组,上颌骨对照组游离骨片应用2个钛接骨板(MNS104)⁃4孔钛板和ψ2.0 mm×7 mm钛钉加以固定;实验组应用相同尺寸钛接骨板(膨胀芯体型)和钛钉加以固定。下颌骨对照组应用钛接骨板(MNS104)⁃4孔钛板和ψ2.0 mm×7 mm钛钉加以固定;实验组应用相同尺寸钛接骨板(膨胀芯体型)和钛钉加以固定。4组均分别于第20天和第40天各取半数(即每组3只)动物处死,应用四环素荧光标记法及组织学观察评价骨折愈合情况。独立样本t检验比较2组间骨折愈合速度的差异。结果:各组动物术后伤口均一期愈合,无明显并发症发生。术后第20天及40天,各组骨折部位表面钛板无松动,固位钛钉稳固,骨折线愈合良好无错位。组织学观察实验组比对照组可见更多的骨小梁和血管增生,其成骨密度较相应的对照组为高。四环素荧光染色提示术后第40天后4组骨折区域均可见新骨的形成;A组平行的相邻两条成骨线距离约为(28.54±1.77)μm;B组为(17.08±1.07)μm;C组为(30.14±5.72)μm;D组为(18.06±2.77)μm;A组平行的相邻两条成骨线距离明显窄于C组,B组明显宽于A组,D组明显宽于C组(P<0.05);而B、D两组未见明显差异(P>0.05),实验组成骨速度显著优于对照组。结论:两种固定方式均能为颌骨骨折提供良好的稳固固定,膨胀芯体型小钛板内固定对上颌骨等游离骨固位更加方便快捷。Objective:To investigate the fixation effect of small titanium plate on two strong internal fixation modes for jaw fracture.Methods:Twenty⁃four hybrid dogs were randomly divided into four groups:A(maxilla control group),B(mandible control group),C(maxilla experimental group)and D(mandible experimental group),and the free bone pieces in the maxillary control group were fixed by two titanium plates(MNS104)⁃4⁃hole titanium plate andψ2.0 mm×7 mm titanium nails.The experimental group was fixed with titanium bone plates of the same size(expansion core type)and titanium nails.The mandible control group was fixed with titanium plate(MNS104)⁃4⁃hole titanium plate andψ2.0 mm×7 mm titanium nails.The experimental group was fixed with titanium bone plates of the same size(expansion core type)and titanium nails.Half of the animals(3 animals in each group)were sacrificed on the 20th and 40th days of the experiment,and the fracture healing was evaluated by tetracycline fluorescent labeling and histological observation.The statistical software package was used to perform a stand⁃alone t⁃test to compare the speed of fracture healing.Results:The fixation effect of the two small tita⁃nium modes could ensure the first⁃stage healing of upper and lower jaw fractures in dogs.On the 20th and 40th days after sur⁃gery,there was no loosening of the titanium plate on the surface of the fracture site in each group,and the fixation titanium nails were stable.The fracture line healed well without dislocation.Histological observation showed that the experimental group showed more bone trabeculae and vascular proliferation than the control group,and bone density was higher than the corre⁃sponding control group.Tetracycline fluorescence staining indicates that new bone formation could be observed in all four groups of fracture areas 40 days after surgery.The distance between the parallel adjacent osteogenic lines in group A was ap⁃proximately(28.54±1.77)μm,group B was(17.08±1.07)μm,group C was(30.14±5.72)μm,group
