不同年龄段孕妇胎儿染色体异常发生情况分析

摘　　要：目的 探索不同年龄段孕妇胎儿染色体异常发生情况,为不同年龄段孕妇选择适宜的产前筛查或产前诊断方案提供更多参考。方法 将2014年1月至2016年12月在南京市妇幼保健院进行妊娠中期唐氏综合征母血清学筛查的44615例单胎妊娠孕妇,根据孕妇预产年龄分为<35岁、35岁、36岁、37岁四组,分析胎儿染色体异常发生率随孕妇年龄的变化趋势及相关性,并比较不同年龄组间胎儿染色体异常情况。结果 <35岁、35岁、36岁和37岁各组胎儿染色体异常发生率分别为0.14%、0.47%、0.57%和0.94%;孕妇年龄与胎儿染色体异常发生率呈正相关,相关系数为0.98,P=0.020;与<35岁年龄组相比,37岁年龄组胎儿染色体异常发生率显著增加,P=0.032;37岁与<37岁相比胎儿染色体异常发生率亦显著增加,P=0.038。结论 随着孕妇年龄增长胎儿染色体异常发生率增加,尤其预产年龄为37岁的孕妇较低龄孕妇胎儿染色体异常发生率增加更为显著,应优先考虑产前诊断方案。Objective To explore the occurrence of fetal chromosomal abnormality in pregnant women of different ages,and provide more references for the selection of appropriate prenatal screening or diagnosis schemes for pregnant women of different ages.Methods A total of 44615 single pregnant women who underwent serological screening of Down syndrome during the second trimester in Nanjing Maternal and Child Health Care Hospital from January 2014 to December 2016 were divided into four groups according to their pre-delivery age:<35 years old,35 years old,36 years old and 37 years old.The changing trend and correlation of the incidence of fetal chromosome abnormality with the age of pregnant woman were analyzed,and the incidences of fetal chromosome abnormality in different age groups were compared.Results The incidence of fetal chromosomal abnormality in<35,35,36 and 37 years old age groups was 0.14%,0.47%,0.57%and 0.94%,respectively.Maternal age was positively correlated with the incidence of fetal chromosome abnormality,with the P value of 0.020 and the correlation coefficient of 0.98.Compared with<35 years old group,the incidence of fetal chromosome abnormality in 37 years age group was significantly increased,with the P value of 0.032.While compared with<37 years old pregnant women,the incidence of fetal chromosomal abnormality was also increased significantly,with the P value of 0.038.Conclusion The incidence of fetal chromosomal abnormality increases with the age of pregnant women,additionally the incidence increases significantly in pregnant woman with pre-delivery age of 37 when compared with that of younger women.Special attention should be paid to this population,and priority should be given to prenatal diagnosis scheme.
