斜视术后干眼症患者泪液炎性因子与泪膜变化的相关性分析

摘　　要：目的 分析斜视术后干眼症患者泪液炎性因子与泪膜变化的相关性。方法 选取浙江省衢州市人民医院2020年5月~2022年5月收治的水平斜视矫正术患者87例(113眼),其中术后干眼症患者42例(55眼)作为干眼组,同期进行斜视术未并发干眼症患者45例(58眼)作为非干眼组。比较两组手术前后泪液中炎性因子IL-1β和IL-18、IL-6、TNF-α水平,检测两组泪膜破裂时间(BUT)、泪液分泌试验(Schirmer I test,SIt)和角膜荧光素染色(fluorescence staining,FL)评分,并分析干眼组泪液各炎性因子与泪膜变化的相关性。结果 术前两组各炎性因子水平与泪膜功能差异均无统计学意义(P>0.05)。斜视术后患者各炎性因子水平较术前升高,干眼组炎性因子水平明显高于非干眼组(均P<0.05);斜视术后两组BUT、SIt较术前显著降低,FL评分较术前升高,差异有统计学意义,干眼组BUT、SIt明显低于非干眼组,FL评分明显高于非干眼组(均P<0.05);干眼组炎性因子IL-1β、IL-18、IL-6、TNF-α与FL呈正相关(r分别为0.794、0.773、0.783、0.789,均P<0.05),炎性因子IL-1β、IL-18、IL-6、TNF-α与BUT呈负相关(r分别为-0.500、-0.593、-0.559、-0.597,均P<0.05);炎性因子IL-1β、IL-18、IL-6、TNF-α与SIt呈负相关(r分别为-0.462、-0.520、-0.502、-0.533,均P<0.05)。结论 斜视手术对患者炎性因子、泪膜功能均有影响,术后干眼症患者炎性因子较高,泪膜功能较差,术后干眼症患者泪液炎性因子IL-1β、IL-18、IL-6、TNF-α与泪膜功能有显著相关性。Objective To analyze the correlation between tear inflammatory factors and tear film changes in patients with dry eye after strabismus surgery.Methods A total of 87 patients(113 eyes)who underwent strabismus surgery in People’s Hospital of Quzhou from May 2020 to May 2022 were selected.Among them,42 patients(55 eyes)with postoperative dry eye were used as the dry eye group,and 45 patients(58 eyes)without dry eye were used as the non-dry eye group.The levels of IL-1β、IL-18、IL-6 and TNF-αin tears before and after surgery were compared between the two groups.The breakup time of tear film(BUT),Schirmer I test(SIt)and corneal fluorescence staining(FL)scores of the two groups were detected,and the correlation between tear inflammatory factors and tear film changes in the dry eye group was analyzed.Results There was no significant difference in the levels of inflammatory factors and tear film function between the two groups before operation(all P>0.05).The levels of inflammatory factors after strabismus surgery were higher than those before surgery,and the levels of inflammatory factors in dry eye group were significantly higher than those in non-dry eye group(all P<0.05).The BUT and SIt of the two groups were significantly lower than those before operation,and the FL score was significantly higher than that before operation.The BUT and SIt of the dry eye group were significantly lower than those of the non-dry eye group,and the FL score was significantly higher than that of the non-dry eye group(all P<0.05).The inflammatory factors IL-1β、IL-18、IL-6 and TNF-αin the dry eye group were positively correlated with FL(r=0.794、0.773、0.783、0.789,all P<0.05),and the inflammatory factors IL-1β,IL-18,IL-6 and TNF-αwere negatively correlated with BUT(r=-0.500、-0.593、-0.559、-0.597,all P<0.05).IL-1β、IL-18、IL-6、TNF-αwere negatively correlated with SIt(r=-0.462、-0.520、-0.502、-0.533,all P<0.05).Conclusion Strabismus surgery has an impact on patients’inflammatory factors and tear film function
