摘 要:目的比较超声乳化白内障吸除术联合不同多焦点人工晶状体(MIOL)植入术后的视觉质量。方法回顾性病例对照研究。纳入山西省眼科医院2018年12月至2021年12月行超声乳化白内障吸除MIOL植入术的白内障42例(60眼),依据患者植入的MIOL分为两组:A组,20例(27眼),植入Tecnis ZMB00 MIOL;B组,22例(33眼),植入ReSTOR+3.0D MIOL。术后1 d、1周、1、3及6个月定期随访,检查患者视力(logMAR)、离焦曲线、对比敏感度、波前像差及问卷调查,并对结果进行统计学分析。结果术后6个月,两组患者矫正远视力、裸眼近视力差异无统计学意义(P=0.056、0.056);A组患者裸眼远视力为-0.1(-0.1,0.0),显著优于B组(P=0.035);B组患者裸眼中视力为0.0(-0.1,0.0),显著优于A组(P=0.046)。在5 mm瞳孔直径下,B组全眼球差和眼内总像差分别为0.06(0.01,0.10)μm、1.64(0.83,3.48)μm,均显著高于A组(P=0.007、0.009),A组眼内球差为-0.13(-0.16,-0.10)μm,显著高于B组(P<0.001)。A、B两组调查问卷总分分别为96.4(83.9,100.0)分、98.2(96.0,99.6)分,差异无统计学意义(P=0.598)。结论超声乳化白内障吸除术联合植入两种类型MIOL植入术后均可获得较好的全程视力,良好的视觉质量以及较高的术后视觉生活质量。Objective To compare the visual quality of patients after phacomulsification with different types of multifocal intraocular lenses(MIOLs)implantation.Methods This was a retrospective case-control study.The clinical data of 60 eyes of 42 patients who underwent phacoemulsification combined with MIOL implantation in Shanxi Eye Hospital from Dec.2018 to Dec.2021 were analyzed.Based on the type of IOL implanted during operation,they were divided into two groups.In group A,27 eyes of 20 cases were implanted with Tecnis ZMB00(Johnson&Johnson Vision,USA).In group B,33 eyes of 22 cases were implanted with ReSTOR+3.0 D(Alcon Laboratories Inc.,USA).The patients were followed up regularly at 1day,1 week,1,3 and 6 months postoperatively for visual acuity(logMAR),defocus curves,contrast sensitivity,wavefront aberration and questionnaires,and the results were statistically analysed.Results There was no statistically significant difference in the preoperative parameters between the two groups(P>0.05).There was no significant difference in best-corrected distance visual acuity and uncorrected intermediate visual acuity between the two groups(P=0.056).The uncorrected distance visual acuity of group A was-0.1(-0.1,0.0),which was significantly better than that of group B(P=0.035).The uncorrected intermediate visual acuity of group B was 0.0(-0.1,0.0),which was significantly better than that of group A(P=0.046).With a 5 mm diameter pupil,the intraocular spherical aberration in group A was-0.13(-0.16,-0.10),which was larger than that of group B(P<0.001)and the total spherical aberration and intraocular aberration in group B were 0.06(0.01,0.10)μm and 1.64(0.83,3.48)μm,respectively,which were larger than group A(P=0.007,0.009).The level of overall satisfaction for groups A and B were 96.4(83.9,100.0)scores and 98.2(96.0,99.6)scores,respectively,and the difference was not statistically significant(P=0.598).Conclusion Phacoemulsification combined with two types MIOLs implantation can achieve better vision,high optical quality and satis
