血清Lp-PLA2、VILIP-1和8-OHDG对高血压脑出血患者预后不良的预测价值

摘　　要：目的探讨血清脂蛋白相关性磷脂酶A2(Lp-PLA2)、视椎蛋白样蛋白1(VILIP-1)和8-羟脱氧鸟苷(8-OHDG)对高血压脑出血患者预后不良的预测价值。方法选择2020年1月至2022年1月该院97例高血压脑出血患者纳入脑出血组。选择同期该院45例健康体检者纳入健康对照组。采用酶联免疫吸附试验检测血清Lp-PLA2、VILIP-1和8-OHDG水平。观察高血压脑出血患者血清Lp-PLA2、VILIP-1和8-OHDG水平与健康对照者的差异,与脑出血量、脑出血严重程度和预后不良的关系,以及其预测高血压脑出血患者预后不良的价值。结果脑出血组血清Lp-PLA2、VILIP-1和8-OHDG水平明显高于健康对照组(P<0.05)。脑出血组血清Lp-PLA2、VILIP-1和8-OHDG水平随着脑出血量和神经缺损严重程度的升高而升高,并且预后不良患者血清Lp-PLA2、VILIP-1和8-OHDG水平明显高于预后良好患者(P<0.05)。血清Lp-PLA2、VILIP-1和8-OHDG水平预测脑出血患者预后不良具有较高的价值,联合检测的灵敏度为95.2%,特异度为71.1%,曲线下面积(AUC)为0.924,明显高于Lp-PLA2(Z=2.490,P=0.013)、VILIP-1(Z=2.240,P=0.025)和8-OHDG(Z=2.434,P=0.015)单独检测,而3项指标单独检测的AUC比较,差异无统计学意义(P>0.05)。结论Lp-PLA2、VILIP-1和8-OHDG参与了高血压脑出血的发生、发展过程,其水平升高与预后不良有关,联合检测有助于提高高血压脑出血患者预后不良的预测效能。Objective To investigate the predictive value of serum lipoprotein-related phospholipase A2(Lp-PLA2),visinin-like protein 1(VILIP-1)and 8-hydroxydeoxyguanosine(8-OHDG)in the poor prognosis in the patients with hypertensive cerebral hemorrhage.Methods Ninety-seven patients with hypertensive cerebral hemorrhage in this hospital from January 2020 to January 2022 were selected as the cerebral hemorrhage group.Forty-five healthy subjects undergoing the physical examination in this hospital during the same period were selected as the healthy control group.The serum Lp-PLA2,VILIP-1 and 8-OHDG levels were determined by the enzyme-linked immunosorbent assay.The differences in the serum Lp-PLA2,VILIP-1 and 8-OHDG levels between the patients with hypertensive cerebral hemorrhage and healthy control,and their relationship with cerebral hemorrhage volume,cerebral hemorrhage severity and poor prognosis were observed,and their value in predicting the poor prognosis of the patients with hypertensive cerebral hemorrhage was also observed.Results The levels of serum Lp-PLA2,VILIP-1 and 8-OHDG in the cerebral hemorrhage group were significantly higher than those in the healthy control group(P<0.05).The levels of serum Lp-PLA2,VILIP-1 and 8-OHDG were increased with the increase of the cerebral hemorrhage amount and neurological defects severity,moreover the levels of serum Lp-PLA2,VILIP-1 and 8-OHDG in the patients with poor prognosis were significantly higher than those in the patients with good prognosis(P<0.05).The serum Lp-PLA2,VILIP-1 and 8-OHDG levels had high value in predicting the poor prognosis in the patients with cerebral hemorrhage.The sensitivity of combined detection was 95.2%,the specificity was 71.1%,and the area under the curve(AUC)was 0.924,which was significantly higher than that of Lp-PLA2(Z=2.490,P=0.013),VILIP-1(Z=2.240,P=0.025)and 8-OHDG(Z=2.434,P=0.015)detection alone,while there was no statistically significant difference in AUC among the three indicators detected alone(P>0.05).Conclusion Lp-PLA2,VILIP-1 an
