基于扎根理论构建彭建中治疗慢性肾脏病的诊疗经验模型

摘　　要：目的:应用扎根理论构建彭建中教授治疗慢性肾脏病的诊疗经验和理论。方法:使用程序性扎根理论方法设计和处理访谈数据,使用Atlas.ti 8.0软件对文字资料进行初级编码,在此基础上抽提核心类别,构建传承模型。结果:将彭建中教授治疗慢性肾脏病的诊疗经验归纳为辨治方法、诊疗技术、用药特点、核心方药共4个类别;轴心式编码阶段重点展示典范模型微分析、条件矩阵微分析;选择性编码阶段抽提出核心类别“同病同治——大法指导下的随证治之”。结论:彭建中教授重视结合现代医学认识疾病,擅长运用中医理论分析肾病病因、病理、预后,临证重视脉舌色症互参,提出“同病同治——大法指导下的随证治之”的诊疗理论,指导肾病的治疗。Objective:To apply the grounded theory to construct professor PENG Jianzhong's experience and theory in the treatment of chronic nephropathy.Methods:This study used the procedural grounded theory to design and process data,and carried out in-depth interviews with one-to-one individuals to collect data.The Atlas.ti 8.0 software was used to encode the interview text and literature of professor PENG Jianzhong,his disciples and patients based on the grounded theory,and then extracted the core categories and constructed the inheritance model.Results:Clinical experience on chronic kidney disease of PENG Jianzhong were obtained as four categories:treatment differentiation,diagnosis and treatment techniques,drug characteristics,and core prescriptions.In the axial coding stage,it took professor PENG Jianzhong's characteristics of drug use as an example to show the exemplary model micro-analysis,and took the application of traditional Chinese medicine theoryunderstanding occult blood as an example to demonstrate the conditional matrix micro-analysis.In the selective coding phase,getting the core category as‘the same diseases use the same means of treatment,following the diagnosis and treatment under the guidance of the law’.Conclusion:Professor PENG Jianzhong advocates combining the specific application of chronic kidney disease with the understanding of the disease in modern medicine,using the theoretical analysis of traditional Chinese medicine and the mutual reference of pulse and tongue disease.‘The same diseases use the same means of treatment,and following the diagnosis and treatment under the guidance of the law’to guide the treatment of kidney disease.
