鼻内镜下治疗双侧上额窦含牙囊肿一例

摘　　要：目的探讨上颌窦含牙囊肿的诊断方法及常见手术方式的优缺点。方法报道一例少见双侧上颌窦含牙囊肿患者的诊断及鼻内镜手术过程,并对上颌窦囊肿诊断方法及常见手术方式进行总结分析。结果上颌窦含牙囊肿少见,较大的病灶会引起压迫性症状,影像学检查及病理检查对于该病诊断具有重要意义,本病例根据病变特点,左侧上颌窦病灶采用鼻内镜中鼻道入路清除,右侧上颌窦病灶采用中鼻道联合下鼻道入路进行清除。结论上颌窦病变的每一种手术方式都存在的优点及缺点,临床应根据病变特点,积极与患者沟通,选择出适合患者的手术。Objective To explore the diagnostic methods of maxillary sinus dental cysts and the characteristics of common surgical methods for maxillary sinus masses.Method This paper reports the diagnosis and endoscopic sinus surgery of a rare patient with bilateral maxillary sinus dental cyst.The diagnostic methods and common surgical methods of maxillary sinus cyst were summarized and analyzed.Result Dental cysts in the maxillary sinus are rare.Larger lesions can cause compressive symptoms.Imaging examination and pathological examination play an important role in the diagnosis of dental cysts.According to the characteristics of the lesions,the lesions of the left maxillary sinus were removed by endoscopic middle nasal meatus approach,and the right maxillary sinus lesions were removed by middle nasal meatus combined with inferior nasal meatus approach.Conclusion Each operation method of maxillary sinus disease has its advantages and disadvantages.According to the characteristics of the disease,we should actively communicate with the patients,and then choose the operation suitable for the patients.
