E6/E7阳性在宫颈病变风险评估及风险分层管理中的价值研究

摘　　要：目的:研究人乳头状瘤病毒(HPV)E6/E7 mRNA检测在宫颈病变风险评估及在宫颈癌筛查中的潜在应用价值。方法:选取2019年1月—2023年1月重庆市璧山区妇幼保健院收治的宫颈病变患者490例作为观察对象,患者均接受HPV E6/E7mRNA及薄层液基细胞学检测,将组织病理学检测结果作为金标准,比较不同程度宫颈病变患者HPV E6/E7 mRNA阳性率及相对表达量。结果:病理分级CIN2及以上患者HPV E6/E7 mRNA阳性患者占比高于HPV E6/E7 mRNA阴性占比,病理分级CIN2以下患者HPVE6/E7mRNA阳性患者占比低于HPVE6/E7mRNA阴性占比,差异有统计学意义(P<0.001)。随着宫颈组织学与细胞学病变程度不断提高,HPVE6/E7mRNA阳性率及拷贝数量均逐渐升高,差异有统计学意义(P<0.001)。结论:HPV E6/E7 mRNA表达与宫颈病变程度存在明显关联,可将该检测作为HPV阳性患者的分流方式。Objective:To study the potential application value of human papilloma virus(HPV)E6/E7 mRNA test in risk assessment of cervical lesions and in cervical cancer screening.Methods:Four hundred and ninety patients with cervical lesions admitted to Chongqing Bishan District Maternity and Child Health Hospital from January 2019 to January 2023 were selected as the observation subjects.The patients were subjected to HPV E6/E7 mRNA and thin-layer liquid-based cytology testing,and the results of histopathologic testing were used as the gold standard to compare the HPV E6/E7 mRNA positivity rate and relative expression in patients with different degrees of cervical lesions.Results:The percentage of HPV E6/E7 mRNA positivity patients was higher than the percentage of HPV E6/E7 mRNA negativity in patients with pathological grading CIN2 and above,and the percentage of HPV E6/E7 mRNA positivity patients was lower than the percentage of HPV E6/E7 mRNA negativity in patients with pathological grading CIN2 or below,and the difference was statistically significant(P<0.001);With the increasing degree of cervical histologic and cytologic lesions,HPV E6/E7 mRNA positivity and copy number were gradually increased,and the difference was statistically significant(P<0.001).Conclusion:There is a significant correlation between HPV E6/E7 mRNA expression and the degree of cervical lesions,and the test can be used as a triage method for HPV-positive patients.
