肌筋膜触痛点局部干预治疗慢性偏头痛效果分析

摘　　要：目的:探讨慢性偏头痛合并肌筋膜触痛点(myofascial trigger points,MTrPs)行局部干预治疗的临床效果。方法:回顾性分析2019年1月—2021年6月南通市第三人民医院神经内科诊治的合并MTrPs的慢性偏头痛患者90例,根据治疗方法的不同分为对照组(布洛芬缓释胶囊口服)、双氯芬酸钠凝胶组(布洛芬缓释胶囊口服联合双氯芬酸钠凝胶局部外用)及干针组(布洛芬缓释胶囊口服联合干针),每组30例。比较3组患者治疗前及治疗1个月后头痛发作频次、视觉模拟评分(visual analogue scale,VAS)及匹兹堡睡眠质量指数量表(Pittsburgh sleep quality index,PSQI)评分。结果:双氯芬酸钠凝胶组及干针组的总有效率分别为93%和90%,高于对照组的70%(P<0.05);双氯芬酸钠凝胶组与干针组PSQI及VAS评分、头痛发作频次均低于对照组(均P<0.05)。结论:MTrPs局部干预治疗能够降低偏头痛患者的头痛发作频次,明显缓解头痛症状,改善患者生活质量,其中常规联合双氯芬酸钠凝胶外用效果显著。Objective:To investigate the clinical effect of local intervention for chronic migraine with myofascial trigger points(MTrPs).Methods:A total of 90 chronic migraine patients with MTrPs diagnosed and treated in the Department of Neurology of Nantong Third People′s Hospital from January 2019 to June 2021 were retrospectively analyzed.According to the different treatment methods,they were divided into control group(oral ibuprofen sustained-release capsule),diclofenac sodium gel group(oral ibuprofen sustained-release capsule combined with topical diclofenac sodium gel)and dry acupuncture group(oral ibuprofen sustained-release capsule combined with dry acupuncture),with 30 cases in each group.The frequency of headache attack,visual analogue scale(VAS)and Pittsburgh sleep quality index(PSQI)scale were compared among the three groups before and after 1 month of treatment.Results:The curative effect of diclofenac sodium gel group and dry acupuncture group were 93%and 90%,respectively,which were higher than 70%of the control group(P<0.05).The PSQI and VAS scores and the frequency of headache attack in the diclofenac sodium gel group and the dry acupuncture group were lower than those in the control group(all P<0.05).Conclusion:Local intervention with MTrPs can reduce the frequency of headache,significantly relieve headache symptoms,and improve the quality of life in patients with migraine,and the conventional combined with diclofenac sodium gel has a significant effect.
