摘 要:目的探讨血清糖类抗原125(CA125)、糖类抗原199(CA199)、人附睾蛋白4(HE4)联合对卵巢附件妇科影像报告与数据系统(GI-RADS)4类肿瘤良恶性的鉴别价值。方法收集2019年2月至2022年6月在濮阳市人民医院进行治疗的卵巢附件GI-RADS 4类肿瘤的116例患者,均经手术或穿刺活检获得病理诊断结果。根据最终病理诊断结果将患者分为良性组和恶性组。观察并比较两组患者血清CA125、CA199、HE4水平以及三者阳性检出率,另绘制受试者工作特征(ROC)曲线分析血清CA125、CA199、HE4联合对卵巢附件GI-RADS 4类肿瘤良恶性的鉴别价值。结果病理诊断结果显示,116例卵巢附件GI-RADS 4类肿瘤患者中,共76例良性肿瘤,40例恶性肿瘤。恶性组血清CA125、CA199、HE4水平均高于良性组(P<0.05);恶性组CA125、CA199、HE4阳性率检出均高于良性组(P<0.05);ROC曲线分析显示,CA125、CA199、HE4联合鉴别卵巢附件GI-RADS 4类肿瘤良恶性的灵敏度、特异度、曲线下面积(AUC)分别为92.50%、85.53%、0.933,联合检测的灵敏度、AUC高于各指标单独检测(P<0.05),联合检测的特异度与各指标单独检测比较差异无统计学意义(P>0.05)。结论CA125、CA199、HE4均对卵巢附件GI-RADS 4类肿瘤良恶性具有一定的鉴别价值,且三者联合鉴别价值更高。【Objective】To investigate the differential value of serum carbohydrate antigen 125(CA125),carbohydrate antigen 199(CA199),and human epididymal protein 4(HE4)in the diagnosis of benign and malignant ovarian appendage Gynecological Imaging Reporting and Data System(GI-RADS)type 4 tumors.【Methods】A total of 116 patients with GI-RADS type 4 ovarian appendage tumors who were treated in Puyang People's Hospital from February 2019 to June 2022 were collected,and pathological diagnosis results were obtained through surgery or puncture biopsy.According to the final pathological diagnosis results,the patients were divided into benign group and malignant group.The levels of serum CA125,CA199,HE4 and the positive detection rate of the three were observed and compared between the two groups.In addition,the receiver operating characteristic(ROC)curve was drawn to analyze the differential value of serum CA125,CA199 and HE4 in the diagnosis of benign and malignant ovarian appendage GI-RADS 4 tumors.【Results】Pathological diagnosis results showed that among 116 patients with GI-RADS type 4 tumors of ovarian appendages,76 were benign tumors and 40 were malignant tumors.Serum CA125,CA199 and HE4 levels in malignant group were higher than those in benign group(P<0.05).The positive rates of CA125,CA199 and HE4 in malignant group were higher than those in benign group(P<0.05).ROC curve analysis showed that the sensitivity,specificity and area under curve(AUC)of CA125,CA199 and HE4 in the combined detection of benign and malignant GI-RADS tumors in ovarian appendages were 92.50%,85.53%and 0.933,respectively.The sensitivity and AUC of combined detection were higher than those of single detection(P<0.05).There was no significant difference between the specificity of combined detection and that of single detection(P>0.05).【Conclusion】CA125,CA199 and HE4 all have a certain value in differential diagnosis of benign and malignant GI-RADS tumors in ovarian appendages,and the combination of the three has higher identification va
