摘  要:目的:探讨术后不同剂量^(131)I治疗对中危分化型甲状腺癌患者清甲成功率及唾液腺功能的影响。方法:回顾性分析2018年3月—2021年2月新余市人民医院和江西省肿瘤医院收治的82例中危分化型甲状腺癌患者,按照^(131)I的治疗剂量将患者分为低剂量组(n=45,50~100 mCi/次)和高剂量组(n=37,101~150 mCi/次)。观察两组患者的清甲成功情况、唾液腺-腮腺的最大浓聚率(MAR)、酸刺激后最大分泌率(MSR)、唾液腺-颌下腺的MAR、酸刺激后MSR及无病生存率。结果:低剂量组有38例患者清甲成功,高剂量组有34例患者清甲成功,低剂量组的清甲成功率(84.44%)低于高剂量组(91.89%),但两组比较,差异无统计学意义(χ^(2)=0.471,P>0.05)。两组治疗后唾液腺-腮腺的MAR、MSR均下降,低剂量组的MAR[(61.18±3.24)%]、MSR[(61.26±4.39)%]均高于高剂量组[(58.46±3.51)%、(57.85±4.19)%],差异均有统计学意义(P<0.05)。两组治疗后唾液腺-颌下腺的MAR、MSR均下降,低剂量组的MAR[(42.62±3.41)%]、MSR[(51.28±3.52)%]均高于高剂量组[(40.46±3.51)%、(49.43±3.37)%],差异均有统计学意义(P<0.05)。两组无病生存率比较,差异无统计学意义(P>0.05)。结论:低剂量的^(131)I不仅可以减轻对唾液腺功能的损伤,而且不影响清甲成功率及无病生存率。Objective:To investigate the effect of different doses of ^(131)I treatment after operation on the success rate of thyroid gland ablation and salivary gland function in patients with intermediate risk differentiated thyroid cancer.Method:A total of 82 patients with intermediate risk differentiated thyroid cancer admitted to Xinyu People's Hospital and Jiangxi Cancer Hospital from March 2018 to February 2021 were analyzed retrospectively and patients were divided into low-dose group(n=45,50-100 mCi/times)and high-dose group(n=37,101-150 mCi/times)according to the therapeutic dose of ^(131)I.The success rate of thyroid gland ablation,the maximum concentration rate(MAR)and maximum secretion rate(MSR)after acid stimulation of salivary gland-parotid gland,the MAR and MSR after acid stimulation of salivary gland-submaxillary gland and the disease free survival rate of the two groups were observed.Result:There were 38 patients in the low-dose group and 24 patients in the high-dose group,and the success rate of thyroid gland ablation of low-dose group(84.44%)was lower than that of high-dose group(91.89%),but there was no significant difference between the two groups(χ^(2)=0.471,P>0.05).After treatment,the MAR and MSR of salivary gland-parotid gland decreased in both groups,and the MAR[(61.18±3.24)%]and MSR[(61.26±4.39)%]in low-dose group were higher than[(58.46±3.51)%,(57.85±4.19)%]in high-dose group,the differences were statistically significant(P<0.05).After treatment,MAR and MSR of salivary gland-submaxillary gland were decreased in both groups,and MAR[(42.62±3.41)%]and MSR[(51.28±3.52)%]in low-dose group were higher than those in high-dose group[(40.46±3.51)%,(49.43±3.37)%],the differences were statistically significant(P<0.05).There was no significant difference in disease free survival rate between the two groups(P>0.05).Conclusion:Low dose of ^(131)I can not only reduce the damage of salivary gland function,but also do not affect the success rate of thyroid gland ablation and diseas free survival rate.
