摘 要:目的:探讨硫辛酸治疗对2型糖尿病周围神经病变(DPN)患者白细胞介素-17A(IL-17A)、单核细胞趋化蛋白1(MCP-1)和网膜素1(Omentin-1)水平的影响。方法:将2015年9月—2020年10月连云港市第一人民医院收治的120例2型糖尿病伴DPN患者按随机数字表法分为治疗组和对照组,各60例。对照组予以常规治疗,治疗组在常规治疗的基础上,给予硫辛酸600 mg,1次/d静滴,疗程为3周。治疗前后比较两组血脂、血糖及炎症因子水平。结果:治疗前两组血糖、血脂和炎症因子水平比较,差异均无统计学意义(P>0.05)。治疗后,与对照组相比,治疗组的肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、IL-17A、MCP-1、超敏C反应蛋白(hs-CRP)和丙二醛(MDA)水平均更低,低密度脂蛋白胆固醇(LDL-C)、Omentin-1水平均更高(P<0.05)。与对照组相比,治疗组总有效率显著更高(P<0.05)。治疗后,治疗组正中神经和腓总神经的运动神经传导速度和感觉神经传导速度较对照组均更快(P<0.05)。结论:硫辛酸可能通过改善Omentin-1的表达,降低IL-17A和MCP-1等炎症因子水平,提高DPN治疗的有效率。Objective:To investigate the effects of Thioctic Acid therapy on the levels of interleukin-17A(IL-17A),monocyte chemotactic protein 1(MCP-1)and Omentin-1 in patients with type 2 diabetic peripheral neuropathy(DPN).Method:A total of 120 patients with type 2 diabetes complicated with DPN admitted to the First People's Hospital of Lianyungang from September 2015 to October 2020 were divided into treatment group and control group according to random number table method,with 60 cases in each group.The control group was given conventional treatment,and the treatment group was given 600 mg Thioctic Acid,once a day of intravenous drip,for 3 weeks.The levels of blood lipid,blood glucose and inflammatory factors were compared between the two groups before and after treatment.Result:There were no significant differences in the levels of blood glucose,blood lipid and inflammatory factors between the two groups before treatment(P>0.05).After treatment,compared with the control group,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),IL-17A,MCP-1,hypersensitive C reactive protein(hs-CRP)and malondialdehyde(MDA)in the treatment group were lower,and the levels of low density lipoprotein cholesterol(LDL-C)and Omentin-1 in the treatment group were higher(P<0.05).Compared with the control group,the total effective rate of the treatment group was significantly higher(P<0.05).After treatment,themotor nerve conduction velocity and sensory nerve conduction velocity of median nerve and common peroneal nerve in treatment group were faster than those in control group(P<0.05).Conclusion:Thioctic Acid may improve the effective rate of DPN treatment by improving the expression of Omentin-1 and reducing the levels of inflammatory factors such as IL-17A and MCP-1.
