玻璃体腔注射雷珠单抗联合激光光凝手术治疗急进型早产儿视网膜病变的效果评估

摘　　要：目的探究玻璃体腔注射雷珠单抗联合激光光凝手术在急进型早产儿视网膜病变治疗中的应用价值。方法将2020年2月至2022年5月开封市儿童医院80例急进型早产儿视网膜病变患儿,随机数字表法分为激光组40例(76眼)和联合组40例(78眼)。激光组采用激光光凝术治疗,联合组采用玻璃体腔注射雷珠单抗联合激光光凝术治疗。比较两组临床疗效,术前及术后3个月房水血管相关因子[血小板衍生生长因子(PDGF)、转化生长因子-β1(TGF-β1)、血管内皮生长因子(VEGF)],眼压,视力及并发症情况。结果联合组总有效率94.87%高于激光组81.58%(P<0.05);术后3个月联合组房水PDGF、TGF-β1、VEGF水平低于激光组(P<0.05);术后3个月联合组眼压低于激光组,裸眼视力高于激光组(P<0.05);联合组并发症总发生率2.56%低于激光组11.84%(P<0.05)。结论玻璃体腔注射雷珠单抗联合激光光凝手术治疗早产儿视网膜病变能增强治疗效果,抑制血管生长,控制眼压,有助于改善患儿视力,且能提高手术安全性。Objective To investigate the value of vitreous cavity injection of ranibizumab combined with laser photocoagulation in the treatment of acutely retinopathy of prematurity(ROP)infants.Methods Eighty children with acutely ROP in Kaifeng Children's Hospital from February 2020 to May 2022 were divided into 40 cases(76 eyes)in the laser group and 40 cases(78 eyes)in the combination group by random number table method.The laser group was treated with laser photocoagulation,and the combination group was treated with vitreous cavity injection of ranibizumab combined with laser photocoagulation.The clinical efficacy,atrial vascular-related factors[platelet-derived growth factor(PDGF),transforming growth factor-β1(TGF-β1),vascular endothelial growth factor(VEGF)],intraocular pressure,visual acuity and complications were compared between the two groups before and 3 months after surgery.Results The overall efficiency of the combination group was 94.87%higher than that of the laser group 81.58%(P<0.05);the levels of PDGF,TGF-β1 and VEGF in the atrial fluid of the combination group were lower than that of the laser group at 3 months after surgery(P<0.05);the intraocular pressure of the combination group was lower than that of the laser group at 3 months after surgery,and the naked visual acuity was higher than that of the laser group(P<0.05).The total complication rate in the combination group was 2.56%lower than that laser group 11.84%(P<0.05).Conclusion Vitreous cavity injection of ranibizumab combined with laser photocoagulation for acutely ROP can enhance the therapeutic effect,inhibit blood vessel growth,control intraocular pressure,help improve the visual acuity of children,and improve the safety of the procedure.
