麻黄升麻汤治疗腰椎融合术后非感染性发热的疗效观察

摘　　要：目的:观察麻黄升麻汤治疗腰椎融合术后非感染性发热的临床疗效。方法:选取腰椎融合术后非感染性发热患者72例,随机分为对照组和治疗组。治疗组术后服用麻黄升麻汤,对照组术后给予物理降温等常规治疗,两组均治疗7 d。比较两组体温变化、退热疗效、中医证候评分、临床总有效率及安全性等指标。结果:术后第1天治疗组与对照组体温无明显差异(P>0.05),术后发热峰值出现在术后第2天,此后各组体温均值逐渐下降,治疗组体温均值较对照组明显下降(P<0.01)。两组退热疗效相比,治疗组为84.8%(28/33)、对照组为50.00%(17/34),治疗组明显优于对照组(P<0.01)。两组中医证候评分比较,治疗组显著优于对照组(P<0.01)。治疗组总有效率为90.9%(30/33)、对照组为67.6%(23/34),治疗组显著优于对照组(P<0.01)。治疗组与对照组安全性的差异无统计学意义(P>0.05)。结论:麻黄升麻汤治疗腰椎融合术后非感染性发热临床疗效显著,安全性好,能降低体温峰值、缩短术后发热时间,改善患者寒热错杂的临床症状,进而缩短患者住院时间,减轻患者经济负担,减少患者术后痛苦,提高患者满意度,值得临床进一步推广应用。Objective:To observe the clinical effect of Mahuang Shengma decoction(麻黄升麻汤)on non-infectious fever after lumbar fusion.Methods:A total of 72 patients with non-infectious fever after lumbar fusion were randomly divided into the control group and the treatment group.The treatment group was given Mahuang Shengma decoction after surgery,and the control group was given physical cooling and other conventional treatments after surgery.Both groups were treated for 7 days.Body temperature change,antipyretic effect,TCM syndrome score,clinical overall effective rate and safety were compared between the two groups.Results:There was no significant difference in body temperature between the treatment group and the control group on the 1st postoperative day(P>0.05),and the peak of postoperative fever appeared on the 2nd postoperative day.After that,the mean body temperature in each group were decreased gradually,and the mean body temperature in the treatment group was decreased significantly compared with that in the control group(P<0.01).The antipyretic effect was 84.8%(28/33)in the treatment group and 50.0%(17/34)in the control group,and the treatment group was significantly better than the control group(P<0.01).The TCM syndrome score in the treatment group was significantly better than that in the control group(P<0.01).The overall effective rate was 90.9%(30/33)in the treatment group and 67.6%(23/34)in the control group,the treatment group was significantly better than the control group(P<0.01).There was no statistical significance in safety between the treatment group and the control group(P>0.05).Conclusion:Mahuang Shengma decoction has significant clinical efficacy and good safety in the treatment of non-infectious fever after lumbar fusion.It can reduce the peak temperature,shorten the postoperative fever time,relieve the clinical symptoms of patients with mixed cold and heat,and then shorten the hospital stay of patients,reduce the economic burden of patients,decrease the postoperative pain of patients,and im
