摘 要:目的 评价微血管血流(MV-Flow)成像鉴别诊断子宫附件良、恶性肿瘤的价值。方法 前瞻性纳入107例附件肿块患者、共115个肿块,经阴道超声采集MV-Flow图像,测量血管指数(VI);绘制受试者工作特征曲线,评价其鉴别诊断子宫附件良、恶性肿瘤的价值。结果 115个子宫附件肿瘤中,33个恶性肿瘤,82个良性肿瘤。MV-Flow条件下,子宫附件恶性肿瘤血流多呈条状、中心型分布,良性肿瘤血流多呈点状分布或少量条状血流呈周围型分布;恶性肿瘤VI明显高于良性肿瘤(P<0.001)。MV-Flow成像鉴别诊断子宫附件良、恶性肿瘤的曲线下面积为0.918;以VI≥7.15为截断值,其诊断子宫附件恶性肿瘤的准确率为89.57%,敏感度为84.85%,特异度为91.46%。结论 MV-Flow成像鉴别诊断子宫附件良、恶性肿瘤具有较高效能。Objective To observe the value of microvascular flow(MV-Flow)imaging for differential diagnosis of benign or malignant uterine appendage tumors.Methods Totally 115 uterine appendage tumors from 107 patients were enrolled prospectively.MV-Flow images were collected under transvaginal ultrasound,the vascular index(VI)of tumors were measured.Then receiver operating characteristic curve was drawn to evaluate the value of VI obtained with MV-Flow imaging for differential diagnosis of benign or malignant uterine appendage tumors.Results Among 115 uterine appendage tumors,there were 33 malignant and 82 benign ones.Under MV-Flow condition,blood flow of most malignant uterine appendage tumors present strip and central distribution,while of most benign ones distributed punctately,or with small amount of strip blood flow distributed in a peripheral pattern.VI of malignant uterine appendage tumors was significantly higher than that of benign ones(P<0.001).The area under the curve of MV-Flow imaging for differentiating malignant or benign uterine appendage tumors was 0.918.Taken VI≥7.15 as the cut-off value,the accuracy,sensitivity and specificity for diagnosing malignant uterine appendage tumor was 89.57%,84.85%and 91.46%,respectively.Conclusion MV-Flow imaging had high efficacy for differential diagnosis of benign or malignant uterine appendage tumors.
