全麻复合硬膜外阻滞麻醉对老年结直肠癌患者麻醉效果及苏醒质量的影响

摘　　要：目的 分析全麻复合硬膜外阻滞麻醉对老年结直肠癌患者麻醉效果及苏醒质量的影响.方法 选取84例行腹腔镜结直肠癌根治术治疗的老年结直肠癌患者,按奇偶数分组法分为两组,全麻组42例,采用单纯静脉全身麻醉;复合组42例,采用全麻复合硬膜外阻滞麻醉;对比两组患者不同时间点血流动力学变化,评价患者苏醒质量.结果 气腹前10 min(T_(1))、气腹后10 min(T_(2))时两组心率、平均动脉压高于麻醉前(T_(0))(P<0.05),且复合组低于全麻组(P<0.05);手术放气5 min(T_(3))时心率、平均动脉压较T_(1)、T_(2)下降(P<0.05),与T_(0)时比较差异无统计学意义(P>0.05).复合组麻醉评价优良率100.0%,高于全麻组的83.33%,术后苏醒时间、气管导管拔除时间及PACU停留时间短于全麻组,与麻醉相关并发症发生率为9.52%,低于全麻组的30.95%(P<0.05).结论 全麻复合硬膜外阻滞麻醉可维持老年结直肠癌患者术中平稳的血流动力学,苏醒时间短,安全性高,值得应用.Objective To analyze anesthesia effect and recovery quality of elderly patients with colorectal cancer under general anesthesia combined with epidural block anesthesia.Method Eighty-four elderly patients with colorectal cancer were selected for laparoscopic radical resection of colorectal cancer.The patients were divided into two groups according to the odd and even grouping method.42 patients in the general anesthesia group were given simple intravenous general anesthesia.General anesthesia combined with epidural block was used in 42 patients in the composite group.The hemodynamic changes at different time points were compared between the two groups to evaluate the waking quality of patients.Results At 10 min(T_(1)) and 10 min(T_(2)),heart rate and mean arterial pressure were higher than T_(0),and the differences were statistically significant(P<0.05),and the composite group was lower than the general anesthesia group(P<0.05);Heart rate and mean arterial pressure decreased at 5 min(T_3) compared with T_(1) and T_2(P<0.05),there was no significant difference compared with T_(0)(P>0.05).The rate of excellent and good anesthesia evaluation in the composite group was higher than that in the general anesthesia group(83.33%),the postoperative recovery time,tracheal catheter removal time and PACU stay time were shorter than those in the general anesthesia group,and the incidence of anesthesia related compli-cations(9.52%) was lower than that in the general anesthesia group 30.95%(P<0.05).Conclusion General anesthesia combined with epidural block anesthesia can maintain the stable hemodynamics in elderly patients with colorectal cancer during operation.
